Controlled Clinical TrialSleep-disordered breathing in recipients of implantable defibrillators.
To examine the prevalence and clinical significance of sleep-disordered breathing in patients with implantable cardioverter defibrillators (ICD). ⋯ Previously undiagnosed CSA is common in ICD recipients. Severely disordered breathing during sleep was more prevalent among patients with CSA than patients with OSA. This prospective, observational study will examine the long-term clinical significance of sleep-disordered breathing in ICD recipients.

Coronary blood flow produced by muscle contractions induced by intracardiac electrical CPR during ventricular fibrillation.
It has been reported that transthoracic electrical cardiopulmonary resuscitation (ECPR) generates coronary perfusion pressures (CPP) similar to manual chest compressions (MCC). We hypothesized that intracardiac ECPR produces similar CPP. ⋯ Intracardiac applied electrical CPR produced observable skeletal muscle contractions, measurable pressure pulses, and coronary perfusion pressures similar to MCC during a brief episode of untreated VF.

Case ReportsObsessive compulsive disorder in a patient with twiddler's syndrome.
Twiddler's or twist syndrome is the twisting of pulse generators around themselves. It may result from mechanical manipulation that can induce the malfunction of the device. In this case, twiddler's syndrome resulted from compulsive checking of the device. ⋯ Two invasive procedures were required to replace the ICD. Psychiatric intervention prevented the recurrence of twiddler's syndrome in this patient for more than 2 years. We believe that preimplant psychiatric assessment should be the rule.

Spatial relationships between the pulmonary veins and sites of complex fractionated atrial electrograms during atrial fibrillation.
Pulmonary vein (PV) isolation is used for the treatment of atrial fibrillation (AF). Complex fractionated atrial electrogram (CFAE) mapping has been introduced to guide AF ablation. However, the spatial relationship between PV and CFAE is not well defined. ⋯ During PV isolation, extending the ablation lesions by up to 10 mm from the PV ostia might cover most high-frequency CFAE around the PV antra. High-frequency CFAE were more often located in the PV ostia in paroxysmal than in persistent AF.

Diabetes does not affect ventricular repolarization and sudden cardiac death risk in patients with dilated cardiomyopathy.
We studied the effects of diabetes on ventricular repolarization parameters and sudden cardiac death in patients with dilated cardiomyopathy (DCM). ⋯ Diabetes appears to increase the risk of heart failure in patients with DCM without affecting ventricular repolarization parameters and sudden cardiac death risk.
