ReviewEvidence of Nervous System Sensitization in Commonly Presenting and Persistent Painful Tendinopathies: A Systematic Review.
Study Design Systematic review. Objectives To elucidate if there is sensitization of the nervous system in those with persistent rotator cuff (shoulder), lateral elbow, patellar, and Achilles tendinopathies. Background Tendinopathy can be difficult to treat, and persistent intractable pain and dysfunction are frequent. ⋯ This evidence is primarily from studies of upper-limb tendinopathy, and caution should be exercised with inference to lower-limb tendinopathy. A physical therapist evaluated a 25-year-old male military trainee, who reported 1 week of left hip pain exacerbated by running and sitting cross-legged. At follow-up, the patient reported unchanged hip pain and new left knee pain. ⋯ Imaging revealed a nondisplaced lateral left-sided sacral stress fracture and left calcaneal stress fracture. ReviewDevelopment of a Review-of-Systems Screening Tool for Orthopaedic Physical Therapists: Results From the Optimal Screening for Prediction of Referral and Outcome (OSPRO) Cohort.
Literature review and cross-sectional study. ⋯ Concise red flag symptom identification appears to be feasible in outpatient orthopaedic physical therapy settings. Future research will determine how this review-of-systems screening tool needs refinement for different patient populations and whether it predicts clinical outcomes or the need for referral to other providers.
