Comparison of intermittent and continuously nebulized albuterol for treatment of asthma in an urban emergency department.
To compare continuously nebulized albuterol with intermittent bolus nebulization of albuterol. ⋯ Continuous nebulization offers no benefit over intermittent therapy in patients with an initial PEFR of more than 200 L/min. In PEFRs of 200 or less, continuous nebulization may decrease admission rate and improve PEFRs when compared with standard therapy.

A randomized, controlled trial of radiograph ordering for extremity trauma in a pediatric emergency department.
The objectives of this study were to determine whether triage nurses using the Brand protocol would order fewer radiographs than would physicians carrying out standard practice procedures, without missing an increased number of joint or bone injuries; the test characteristics and the interobserver reliability of the Brand protocol; and whether having triage nurses order radiographs could reduce total patient waiting time in the emergency department. ⋯ Having triage nurses use the Brand protocol reduced the number of radiographs ordered but at the same time increased the number of missed radiographic findings. However, having triage nurses order radiographs also significantly shortened waiting time in the ED.

To compare the tissue adhesive Histoacryl Blue with suturing in the repair of pediatric facial lacerations. ⋯ Histoacryl Blue is a faster and less painful method of facial laceration repair that has cosmetic results similar to the use of sutures.

Effect of needle changing and intravenous cannula collection on blood culture contamination rates.
We tested the hypotheses that blood culture positivity and contamination rates were not increased by not changing needles between venipuncture and inoculation of blood culture bottles or by taking blood for culture by freshly inserted IV cannulae. ⋯ The results of this study do not support the practice of changing needles before inoculating blood samples into blood culture bottles. Collection of blood for culture through freshly inserted IV cannulae is associated with a low contamination rate and is an acceptable alternative to direct venipuncture. Sources of bias in this study suggest that further research is needed to determine the optimal technique for collecting blood cultures.
