Comparative StudyStereotactic comparison among cerebral blood volume, methionine uptake, and histopathology in brain glioma.
Vascularity, metabolism, and histologic grade are related in gliomas but the exact determinants of these relationships are not fully defined. We used image coregistration and stereotactic biopsies to regionally compare cerebral blood volume (CBV) and (11)C-methionine (MET) uptake measurements in brain gliomas and to assess their relationship by histopathologic examination. ⋯ Within glial tumors, there is a local relationship between CBV and MET uptake measurements. Both provide indices of focal malignant activity.

Case ReportsAssociation between lumbar epidural injection and development of acute paraparesis in patients with spinal dural arteriovenous fistulas.
We report 3 patients with previously undiagnosed spinal dural arteriovenous fistulas (SDAVFs), who developed acute paraparesis following lumbar epidural steroid injection. MR imaging demonstrated spinal cord T2 hyperintensity, edema and/or enhancement of the conus, and intradural enlarged vascular flow voids. Spinal angiography confirmed SDAVFs arising from pedicles remote from the sites of the epidural steroid injection. Fistulas were eliminated with either endovascular or combination endovascular and open surgical approaches, with subsequent partial resolution of paraparesis.

Case ReportsIdiopathic hypertrophic spinal pachymeningitis: report of two cases with typical MR imaging findings.
Two patients with a chronic progressive myelopathy were successfully surgically treated and idiopathic hypertrophic spinal pachymeningitis (IHSP) was found on histology. In both patients, an extensive extramedullary mass of low T2 signal with peripheral contrast enhancement was compressing the spinal cord on MR imaging. This imaging appearance in patients with chronic progressive myelopathy should suggest the diagnosis of IHSP.

Clinical TrialCorrelation of MR imaging findings with intraoperative findings after cervical spine trauma.
There are limited data correlating MR imaging and anatomic findings of ligamentous injury in cervical spine trauma. This study compares acute MR imaging with surgical observations of disk/ligamentous injury after blunt cervical trauma. ⋯ In acute cervical spine trauma, MR imaging has moderate to high sensitivity for injury to specific ligamentous structures but limited agreement between specific MR imaging findings and injury at surgery. MR imaging may overestimate the extent of disruptive injury when compared with intraoperative findings, with potential clinical consequences.

Comparative Study Clinical TrialComparison of the temporary diagnostic relief of transforaminal epidural steroid injection approaches: conventional versus posterolateral technique.
Conventional transforaminal epidural steroid injection (TFESI) has several problems. The purpose of this study was to compare the temporary diagnostic relief and advantages of TFESI performed using the conventional and posterolateral approaches. ⋯ Our findings suggest that the posterolateral approach is an alternative method for TFESI in cases where needle tip positioning in the anterior epidural space is difficult.
