To investigate changes in corneal topography and morphology in human eyes wearing orthokeratology (OK) lenses for overnight and over a 5-year period. ⋯ Short-term and long-term OK lens wear can effectively correct myopia by flattening the cornea. The flattened cornea in the short-term lens wear is mainly because of the thickening of the midperipheral cornea. In the long-term lens wear, however, this change is associated with thickening of the midperipheral cornea and thinning of the central corneal epithelium.

To determine the efficacy and postoperative complications of complex deep lamellar endothelial keratoplasty (DLEK) when used for the management of complex bullous keratopathy with severe vision loss. ⋯ In cases of complex bullous keratopathy with severe vision loss, DLEK combined with other intraocular surgeries is a feasible and effective procedure to significantly improve visual acuity to a functional level in most patients. DLEK can be used successfully in eyes with aphakia and other anterior segment abnormalities without a high risk of graft dislocation or failure. Mild postoperative complications usually occurred early after DLEK and did not affect visual outcomes and graft survival if managed appropriately.

Case Reports
Salzmann nodules -- a possible new ocular manifestation of Crohn's disease: a case report.
The purpose of this article was to describe an association between Salzmann nodules and Crohn's disease. ⋯ Salzmann nodules may represent a distinct clinical sign of ocular involvement in Crohn's disease.

Review Case Reports
Management of acute hydrops with perforation in a patient with keratoconus and cone dystrophy: case report and literature review.
To describe a patient with cone dystrophy who presented with acute hydrops and perforation, leading to the diagnosis of keratoconus. ⋯ Although the association between keratoconus and cone dystrophy is extremely rare, our patient's vision-threatening complication of acute hydrops with corneal perforation highlights the importance of corneal evaluation including topography in cone dystrophy. Conservative management was successful in the restoration of anatomic integrity in this situation.

Comparative Study
The value-based medicine comparative effectiveness and cost-effectiveness of penetrating keratoplasty for keratoconus.
To perform a base case, comparative effectiveness, and cost-effectiveness (cost-utility) analysis of penetrating keratoplasty for patients with severe keratoconus. ⋯ Penetrating keratoplasty for patients with severe keratoconus seems to be a comparatively effective and cost-effective procedure when compared with other interventions across different medical specialties.
