Review Case Reports[Combination of ulnar head fracture with undisplaced radial neck fracture: case report and review of the literature].
We present a case of a to our knowledge never described before combination of a displaced ulnar head--with an undisplaced radial neck fracture associated with a clinical instability of the distal radioulnar joint (DRUJ). We discuss the management of our atypical case and review the literature.

Comparative Study[Ultrasonic and radiographic quantification of palmar angulation in metacarpal IV and V neck fractures].
Therapy of metacarpal neck fractures depending on radiographically measured palmar angulation is discussed controversially in the literature. Some authors describe normal hand function of malunited metacarpal neck fractures with a palmar angulation up to 70°; others define 30° as the uppermost limit to maintain normal hand function. However, the methods of measuring palmar angulation are not clearly defined. Here, we present a new method to measure palmar angulation using ultrasound. The aim of this prospective study is to compare the radiographic methods of measuring palmar angulation with the ultrasound method. ⋯ The ultrasound method for measuring palmar angulation in metacarpal neck fractures is simple, and appears to be more precise in comparison to radiographic methods. An exactly lateral projection of the fractured metacarpal bone can be imaged without superposition and exposure to radiation. The question arises whether previous studies that suggested normal hand function with palmar angulation up to 70° might have been based on too high angle-readings due to the use of radiographic methods.

Since the introduction of Diagnosis Related Groups (DRGs) in 2004 in Germany the variables of remuneration changed continuously. Subjectively, reimbursement of DRG in hand surgery has a negative connotation among colleagues. We analyzed the development of reimbursement as well as the length of stay of inpatients over time in Hand Surgery considering various parameters concentrating on trends and correlation with macroeconomic parameters. ⋯ For the latter, a surplus of 244 000 Euros and 311 000 Euros were calculated, respectively. No correlation with the Gross National Product or the Rate of Inflation could be found. The Pay Base Rate as the rate of effective payment in our clinic declined by 7% whereas the consumer price index gained 8,6% resulting in a loss of purchasing power of almost 16% in a 6-year period.
