Patellar tendon versus quadrupled bone-semitendinosus anterior cruciate ligament reconstruction: a prospective clinical investigation in athletes.
Patellar tendon and hamstrings are both used in anterior cruciate ligament (ACL) reconstruction, and comparisons have been reported with different results. The purpose of this clinical study was to compare the results of ACL reconstruction in athletes with 2 different graft types, both using bone-to-bone healing: bone-patellar tendon-bone graft and a quadrupled bone-semitendinosus graft. ⋯ The bone-patellar tendon-bone and quadrupled bone-semitendinosus autograft provide excellent grafts for ACL reconstruction. Both techniques are comparable regarding final stability, but in patients with extensor mechanism problems or those who engage in sports with a high incidence of patellar tendonitis, the semitendinosus graft should be considered.

Detachment of the tip of a motorized shaver within the knee joint: a complication of arthroscopic surgery.
In arthroscopic procedures, instrument failures are rare complications with low incidences. We present a case of detachment of the tip of the motorized shaver within the knee joint during an arthroscopic anterior cruciate ligament reconstruction procedure. ⋯ We did not realize that the tip was missing within the knee joint during the surgery. We believe that the design of the shaver blade that was used needs modification, and surgeons must take special precautions to inspect the instrument before and after placing it in the joint.

To evaluate the load to failure and the mode of failure of a novel suture anchor construct that does not require knots (the "twist-lock" construct) and to compare it with a standard suture anchor construct (Corkscrew; Arthrex, Naples, FL). ⋯ The twist-lock system is a suture anchor system that achieves suture fixation of soft tissue to bone without the need to tie knots. It shows single-pull loads to failure that are significantly higher than those of a standard suture anchor system.
