Dr John J. Conley was an integral part of the house of surgery in the latter part of the 20th century. Conley placed the ethical values of practice at the forefront of his teachings and transcribed many valuable lessons in his writings. ⋯ Those principles are revisited here as a way to celebrate the joy of the calling that is medical practice. In addition, 7 new elements are added as a way to enhance that joy in the light of 21st-century medicine. The uplifting experience found in a career filled with ethical conduct is the legacy we should all strive to achieve.

Comparative Study Clinical TrialThe effect of prilocaine or levobupivacaine infiltration on pain during nasal packing removal.
The aim of this study was to evaluate the efficacy of rehydration of Merocel nasal packs with prilocaine or levobupivacaine on reducing pain and discomfort of nasal packing removal in patients who had undergone septoplasties or endoscopic sinus surgery. ⋯ Levobupivacaine or prilocaine infiltration before removal of nasal packs in patients who undergo septoplasties or endoscopic sinus surgery can decrease discomfort and improve patient tolerability.

Comparative StudyIntratympanic dexamethasone to prevent cisplatin ototoxicity: a guinea pig model.
To determine whether intratympanic administration of dexamethasone reduces ototoxicity from systemic cisplatin. ⋯ Modest intratympanic dexamethasone otoprotection of the guinea pig ear was greatest at the highest concentration tested and occurred in a frequency-dependent manner. Intratympanic dexamethasone presents as a safe, simple, and effective treatment modality to minimize cisplatin ototoxicity without interfering with the chemotherapeutic effects of cisplatin.
