Details of piriformis syndrome, including the proper diagnosis and most effective form of treatment, continue to be controversial. While the cause, diagnosis, and treatment of piriformis syndrome remain elusive, many studies have been conducted to investigate newly developed diagnostic techniques as well as various treatment options for piriformis-induced sciatica. ⋯ Thus, despite the evidence supporting the newer treatment methodologies for piriformis syndrome, research should continue. It is important not only to evaluate treatment outcomes based on associated pain relief, but also to investigate the functional and anatomical return that patients experience from these studied treatments in order to fully explore the most effective form of therapy for piriformis syndrome.

Unilateral agenesis of the internal carotid artery presented as transient ischaemic attack: a case report.
Dysgenesis of the internal carotid artery (ICA) is a rare vascular disorder with a variety of different grades (agenesis, aplasia, and hypoplasia). The left internal carotid artery is reported to be affected by dysgenesis three times more often than the right one. Most of the patients with dysgenesis of the internal carotid artery are asymptomatic. ⋯ MR imaging of the brain revealed signal void of the intracranial portion of right internal carotid artery. Maximum intensity projection reconstruction confirmed the agenesis of the right ICA, with the right middle cerebral artery fed through a dilated posterior communicating artery and the right anterior cerebral artery supplied by the anterior communicating artery (fetal type of collateral flow). In patients with agenesis of the internal carotid artery non-invasive imaging techniques are currently the mainstay of diagnosis.

The exhibitions of plastinated cadavers and organs have attracted millions of visitors globally, while raising serious controversy about their content and purpose of implementation. ⋯ The desire for qualitative-guided anatomy education is evident from the highly popular plastinated body and specimen exhibitions. Hence, additional focused effort could be provided to educate the public about normal and pathological anatomy in order to amend their life-style. This could be effected by certified anatomy demonstrators in graduated steps according to the cohort's age, education, occupation, and health status.

Cervical translaminar screw fixation has been shown to be safe, efficient and provides alternative for cervical fixation. However, its use in the Asian population should be considered cautiously because the cervical lamina diameter may not be adequate to accommodate the standard lamina screw size. We studied the average transverse lamina diameter of the cervical spine in the Malaysian population to evaluate the feasibility and safety of lamina screw fixation in this population. ⋯ Translaminar fixation using a 3.5 screw should be attempted with caution in Asian population. Pre-operative CT scan evaluation is mandatory before translaminar screw fixation is attempted.

Studies of sacral pedicle anatomy have been reported in the European population. However, the feasibility for the use of S1, S2 and S2-ilium screws has not been fully investigated in the Asian population. ⋯ The application of S1, S2 and S2-ilium screws are feasible. The amount of medial angulation and the ideal screw length in the Asian population must be borne in mind during insertion. Right S1 screws carry higher risk of injury to the internal iliac artery when the anterior cortical penetration occurs due to the course of the iliac vessels.
