[Prevention of migraine using flunarizine (results of a pilot study using Amalium)].
A short episode of focal cortical hypoxia seems to be the turning-point in the genesis of the migraine attack. This pathophysiological situation induces vascular changes according to the hypothesis of Wolff. Under such conditions some Ca-antagonists develop antihypoxic and antivasoconstrictive properties. ⋯ Treatment was well tolerated. Weight gain was observed in 20.3% of patients likely correlating with the therapeutic efficiency. Due to its efficiency, safety and its long-lasting therapeutic effect, Flunarizine appears to be a very suitable agent in the prophylaxis of migraine.

Diabetics mostly display a deterioration of blood flow properties. Studies of various hemorheological factors in 163 diabetics (96 females, 67 males; 52 subjects with uncomplicated and 49 with proliferative retinopathy) disclosed in comparison to 79 matched controls: Concentrations of macromolecular proteins, especially of fibrinogen, were raised independently of actual diabetic metabolic situation; this was linked with an increase in plasma viscosity and in erythrocyte aggregation, specially pronounced in patients with renal affection. Erythrocyte flexibility assessed by filtration testing was deteriorated in dependence of metabolic control, impairment increasing in patients with progressing diabetic proliferative retinopathy. ⋯ Risk factors such as hypertension, arteriosclerosis and smoking have an aggravating effect. The complex disturbances of blood fluidity can contribute to the development of microcirculatory blood flow disorders, whereby, of course platelet hyperreactivity and hypercoagulation come into play, too. Especially a good diabetes control is a premise for an efficient blood flow promoting rheologic therapy.

[Surgical therapy of coronary disease: aortocoronary bypass. Risks and complications].
The aorto-coronary bypass surgery is a well established method for treating patients with coronary artery disease. In the last five years at the University Clinic in Innsbruck 417 operations have been performed. The operation letality decreased down to 0.66%, whereby the mean revascularisation rate increased to 2.8. The specific aim of this paper is to show the benefits and the risks of this operation to validate and to verify the benefits in regard to the risks, which could be kept very low.

[Pneumatic cold cap for the prevention of doxorubicin-induced alopecia].
Since 1973 several methods were described in the literature to prevent Doxorubicin induced alopecia. Besides the application of pressure scalp hypothermia gained increasing importance. ⋯ We are introducing a method in which a pneumatic cap is used for the application of ice on the scalp. With this method which is comperatively cheap and easy to be handled we got a protective effect against doxorubicin-induced hair loss which was at least as efficient as the current results with other methods referred in the literature.

[Esophageal obturator tube, a new method for emergency artificial respiration].
Since several years, there are reports from the USA about the esophageal obturator airway, a simple device for emergency ventilatory support in the unconscious patient. The simplicity of its use is of advantage and may easily be achieved by paramedics too (blind insertion into the esophagus without the need of a laryngoscope). ⋯ Still, preference should be given to endotracheal intubation whenever feasible. Several modifications of the EOA are discussed as well as possible complications.
