Case ReportsTwin pregnancy with a complete hydatidiform mole and co-existent live fetus: two case reports and review of the literature.
The aim of this study was to report the clinical features, management, and outcome of two cases of complete hydatidiform mole with a coexisting viable fetus and to review the literature. ⋯ The chances of a live birth have been estimated between 30 and 35% and the risk of persistent trophoblastic disease is similar to singleton molar pregnancies in complete mole with coexisting fetus pregnancy. Therefore, in these pregnancies, expectant management instead of termination of pregnancy can be suggested.

To compare the effects of letrozole and clomiphene citrate (CC) on endometrial response in PCOS using subendometrial spiral artery Doppler. ⋯ The effect of letrozole showed a significantly better endometrial response compared to CC.

Randomized Controlled TrialCombined spinal-epidural analgesia for labor pain: best timing of epidural infusion following spinal dose.
The combined spinal-epidural analgesia (CSEA) technique for labor pain has attained wide spread popularity in obstetric anesthesia. The onset of analgesia is rapid and reliable, and maternal satisfaction is very high. However, the best timing of an epidural infusion following the spinal dose and its effect on the total local anesthetics consumption has not been well determined. ⋯ Our results suggest that the best timing of epidural infusion following spinal dose was within 30 min of spinal induction dose.

Letter Case ReportsTisseel for management of traumatic postpartum haemorrhage.
Following the introduction of haemostatic agents, new opportunities are available when facing problematic bleeding. We report the first case of traumatic postpartum hemorrhage successfully controlled with a fibrin sealant. ⋯ Tisseel consists of a two-component fibrin biomatrix to stop diffuse bleeding. When the tissues are very oedematous, it can be difficult to insert sutures. We tried this innovative option with very effective results. The absence of long-term complications is reassuring and we recommend its use in similarly extreme situations.
