Randomized Controlled Trial Comparative StudyPostoperative arrhythmias after coronary artery bypass grafting: a comparison between 'off pump' and 'on pump' CABG.
Coronary artery bypass without cardiopulmonary bypass (CPB) has gained popularity recently with the development of devices that allow for improved exposure and standardization in off pump coronary artery bypass surgery. Off pump coronary artery bypass surgery reduces some of the morbidities traditionally attributed to CPB. The primary goal of off pump coronary artery bypass surgery is to provide an equally comprehensive operational result as the conventional operation. The purpose of the study was to compare intra operative and postoperative arrhythmias in off pump coronary artery bypass (OPCAB) verses conventional CABG. ⋯ There is non-significant tendency towards less frequency of postoperative arrhythmias in the off pump patients as compared to on pump coronary artery bypass surgery patients.

There are many ways to treat aseptic non-union of femoral shaft fractures with reported varied success rate. Amongst all these, Exchange nailing is the simplest and most successful technique for treating aseptic non union of femoral shaft fractures. We have carried out a prospective study in the Department of Trauma & Orthopaedics, Ayub Medical College Abbottabad to analyse the role of exchange nailing for aseptic non union of femoral shaft fractures. ⋯ Exchange nailing is a simple technique for treating aseptic non union of femoral shaft fractures. Based on the results of our study, we recommend it as the procedure of choice for non comminuted, aseptic non union of femoral shaft fracture.

Puerperal sepsis is one of the leading causes of preventable maternal morbidity and mortality. It is still ranked as 3rd major cause of maternal deaths in our country. The objective of this study was to evaluate morbidity and mortality from puerperal sepsis and to identify its risk factors. ⋯ Puerperal sepsis is an important public health problem contributing to maternal morbidity and mortality. Majority of predisposing factors are preventable. Optimal antiseptic measures and careful monitoring are needed throughout the process of labour.

Fast tracking in adult cardiac surgery at Pakistan Institute of Medical Sciences.
Early extubation after cardiac operation is an important aspect of fast-track cardiac anaesthesia. The length of stay in ICU limits utilisation of operation theatre in cardiac surgery. Increasing cost, limited resources, and newer surgical strategies have stimulated effectiveness of all routines in cardiac surgery, anaesthesia, and intensive care. Aim of this study was to determine the feasibility of fast-tracking in adult cardiac surgery and its effects on post operative recovery in our setup. ⋯ Fast-tracking with extubation within 6 hours is feasible approach which minimises the post-operative complications significantly in adult cardiac surgical patients.
