Comparing Ripasa Score And Alvarado Score In An Accurate Diagnosis Of Acute Appendicitis.
The clinical diagnosis of acute appendicitis remains difficult despite the use of different scoring systems. A high rate of negative appendectomies is no longer acceptable. This study was aimed to compare RIPASA score and Alvarado score in Pakistani population for the diagnosis of acute appendicitis using histopathology as a gold standard. ⋯ RIPASA scoring system is a more accurate, sensitive and specific indicator of diagnosing acute appendicitis as compared to Alvarado scoring system.

Observational StudyRamadan-Associated Circadian Census Variation In A Busy Middle Eastern Emergency Department.
An important goal for the Emergency Department operations is planning for changes in patient volume and assuring staffing accordingly. We hypothesized that understanding the Emergency Department census changes during the month-long Ramadan holiday each year could facilitate operations planning for Emergency Department's serving a largely Muslim population. ⋯ As compared to non-Ramadan baseline, Ramadan was associated with substantial changes in overall census and in proportions of various patient subgroups. Therefore, Emergency Department's serving large Muslim population should undertake studying major operations changes that can be expected for the month.

Case ReportsA Breath Close To Death: A Case Report Of Idiosyncratic Reaction To Welding Fumes.
A 39-year-old gentleman presented to emergency department with a few hours' history of acute shortness of breath, cough and haemoptysis that developed whilst welding a steel tank in a closed container. He was welder by profession for thirteen years with no significant past medical history. The arterial blood gas showed severe oxygenation impairment and he was intubated for mechanical ventilation. ⋯ Welding metal fumes related ARDS remains the diagnosis of exclusion and all other causes must be ruled out. The key to treatment is ventilation support with early endotracheal intubation. Appropriate precautionary measures are advised to avoid occupational health hazards in welding profession.

Case ReportsGreen Urine And Altered Mental Status: A Case Of Intoxication.
Green coloured urine is atypical as it usually signifies the presence of an exogenous substance. Several substances in literature have been associated with green urine including propofol, biliverdin, metoclopramide, methylene blue, indigo blue, amitriptyline, methocarbamol, indomethacin, promethazine, cimetidine and food colourings. We present here a case of middleaged man who presented to our ER with altered mental status and green coloured urine with positive urine toxicology reports for benzodiazepine.

Early Diagnosis Of Acute Kidney Injury By Urinary Neutrophil Gelatinase Associated Lipocalin In Adult Critically Ill Patients.
Acute kidney injury (AKI) is a major cause of mortality and morbidity in the hospitalized patients. It is also a risk factor for chronic kidney disease and advance renal failure. Early diagnosis with new biomarkers for AKI can prevent and/or reverse the process before rise in serum creatinine and symptomatic renal failure. This study is aimed at the accuracy of Urine neutrophil gelatinase associated lipocalin (NGAL) for detection of AKI at an early stage. ⋯ Urine NGAL is an accurate marker of AKI in critically ill patients. Therefore, it should be included in the diagnostic workup of AKI in early stages..
