Clinical TrialEffectiveness of combined thoracic epidural and light general anaesthesia in patients undergoing non-cardiac thoracic surgery.
The specialty of thoracic anaesthesia has emerged as a scientifically based discipline just 30 years back. At the start of the 20th century empyema and tuberculosis were the main indications for the thoracic surgery. Later on with the introduction of antibiotics lung malignancies were more commonly operated. Recent resurgence of tuberculosis and associated medical illnesses put these patients in high risk for surgery and anaesthesia, necessitating introduction of more skillful approach. The objective of this study was to evaluate the effectiveness of combined thoracic epidural anaesthesia and light general anaesthesia in patients undergoing non-cardiac thoracic surgery. ⋯ Combined use of light general anaesthesia and thoracic epidural is effective in patients undergoing non-cardiac thoracic surgery.

Magnesium sulphate in the prophylaxis and treatment of eclampsia.
Magnesium Sulphate is considered to be the agent of choice for the control of eclamptic seizures in pregnant women. Our objectives were to determine frequency of eclampsia and pre-eclampsia in our unit and to determine the effect of initial loading dose of magnesium sulphate on maternal and fetal outcome. ⋯ Frequency of eclampsia and pre-eclampsia is high in this region with high perinatal and maternal morbidity and mortality. Magnesium sulphate is an effective drug to prevent and control seizures. It is easy to administer and subsequent nursing is easy. Seizures usually terminate after the initial loading dose of magnesium sulphate.

Accuracy of urine dipstick to predict urinary tract infections in an emergency department.
Urine dipstick is a useful and commonly used test in the Emergency Department because of its rapidity and low cost; however its diagnostic accuracy is debatable. Our objective was to compare the urine dipstick and urinalysis for Urinary Tract Infection in a developing country, where there are significant cost considerations. ⋯ Dipstick alone cannot accurately predict urinary tract infection in emergency department.

Medical profession is still the most sought after profession, there are thousands of bright students who pass their F. Sc examination in First Division and desire to join medical colleges but are unable to get admission in government medical colleges. There is a great shortage of doctors in the country. ⋯ Those colleges which fulfill the laid down requirement of PMDC, help in overcoming the shortage of doctors in the country and provide much needed health care to the population. Such medical colleges are much appreciated by the public and by the govt. Such private sector medical colleges have great future and some of them may become famous and outstanding like the private sector medical colleges in the USA.

Efficacy, safety and tolerability of streptokinase in multiloculated empyema.
Empyema thoracis is a common illness with significant morbidity and mortality. Standard treatment of Empyema includes tube drainage and antibiotics. But the tube drainage often fails if the fluid is loculated. Intrapleural Streptokinase has been used in multiloculated empyemas with good success rate. We evaluated the efficacy and safety of intra-pleural Streptokinase in loculated empyemas. ⋯ Intrapleural Streptokinase is a safe and effective means of increasing the tube drainage in multiloculated Empyema without causing systemic fibrinolysis.
