Case Reports[Vallecular cyst as a rare cause of progressive inspiratory stridor].
Vallecular cysts are frequently observed, benign masses of the upper aerodigestiv tract. Usually they appear as harmless and asymptomatic and go unnoticed by the patient. In rare cases, obstructions of the upper airway due to monstrous vallecular cysts may become life threatening. ⋯ Vallecular cysts that increase in size can cause dyspnea due to obstruction of the aditus of the larynx. This case demonstrates the need for surgical treatment at an early stage of pedicled vallecular cysts.

Comparative Study[Intraoperative bronchospasm during paranasal sinus surgery -- indicator of aspirin intolerance syndrome?].
The aim of this study was to evaluate whether an intraoperative bronchospasm is more frequent in sinus surgery than in non-sinus surgery, whether its appearance after application of a non-steroidal anti-inflammatory drug (NSAID) is an indicator of an aspirin intolerance syndrome, and whether its appearance can be interpreted as an aspirin provocation test. ⋯ An intraoperative bronchospasm during sinus surgery is not a clear indicator of an aspirin intolerance syndrome. An analgesic-induced bronchospasm can also be observed after paracetamol and metamizole. It can not be interpreted analogous to an aspirin provocation test.

Nasal irrigation has proven in several recently published articles its clinical effectiveness and its neglectable rate of side effects even in children and during pregnancy. In most guidelines for the treatment of various nasal pathological conditions nasal irrigation is recommended as stand alone or as additional therapy. ⋯ In children, nasal irrigation can save antibiotics and reduce decongestant to nil. Important is the hygienic application in order not to introduce additional pathogenic germs into the nasal system.

[ENT relevant changes in the German OPS Procedure Classification System for 2006].
Classification of surgical and medical procedures is of increasing relevance for health care financing. From this viewpoint, classification according to the German "Operations- und Prozedurenschlüssel" (OPS) was frequently inadequate so far. ⋯ Significant modifications of the OPS will take place at January 1 (st) 2006, which are of great impact for health care financing by the G-DRG-System and in the context of outpatient surgery.
