ReviewFusion techniques for adult isthmic spondylolisthesis: a systematic review.
Various fusion techniques have been used to treat lumbar spine isthmic spondylolisthesis (IS) in adults, including anterior lumbar interbody fusion (ALIF), posterior lumbar interbody fusion (PLIF), transforaminal lumbar interbody fusion (TLIF), posterolateral fusion (PLF), and circumferential fusion. The objective of this study was to evaluate which fusion technique provides the best clinical and radiological outcome for adult lumbar IS. ⋯ Compared to other fusion techniques, TLIF shows fewer complications, ALIF shows better sagittal alignment, and circumferential fusion showed better fusion rates. It was difficult to make recommendations about the optimal approach because of the methodological variance in the publications.

Randomized Controlled TrialIlizarov external fixation or locked intramedullary nailing in diaphyseal tibial fractures: a randomized, prospective study of 58 consecutive patients.
The aim of this study was to compare the Ilizarov circular fixator (IL) and locked intramedullary nailing (IM). ⋯ The IL is a safe and reliable alternative to IM for the treatment of tibial shaft fractures, with a low complication rate and good clinical outcome. Both treatments were well tolerated, but at the 1-year follow-up the patients in the IM group had more pain and were less satisfied. Finally, there was a high frequency of anterior knee pain in the IM group.

Reliability and validity of carpal alignment measurements in evaluating deformities of scaphoid fractures.
Several radiographic carpal alignment indices are used to evaluate the deformities of scaphoid fractures. The purpose of this study was to determine the reliabilities and validities of radiographic carpal alignment indices commonly used to evaluate deformities of scaphoid fractures. ⋯ Among radiographic carpal alignment measures, radiolunate angle is the most reliable and valid carpal alignment index for evaluating deformities of scaphoid fractures. Scapholunate and capitolunate angles could be used as an alternative, but have less validity.

The effect of trochlea tilting on patellofemoral contact patterns after total knee arthroplasty: an in vitro study.
Patellofemoral complications are one major concern after total knee arthroplasty (TKA). Anterior knee pain is one of these complications and to a high percentage responsible for unsatisfied patients after TKA. Malrotation of the femoral component can contribute to retropatellar peak pressure and consequently to anterior knee pain. ⋯ The implantation of the femoral component by 3° internal trochlea rotation to transepicondylar line resulted in a highly significant increase of the mean maximal retropatellar pressure compared to 6° external rotation of the trochlea of the femoral component (p < 0.01). A higher retropatellar pressure might lead to anterior knee pain after TKA. We recommend an external rotation of the femoral component between 3° and 6° to anatomical transepicondylar line to reduce the maximal retropatellar pressure, but only if adequate soft tissue balancing and stable knee kinematics are provided.
