Association between dementia and postoperative complications after hip fracture surgery in the elderly: analysis of 87,654 patients using a national administrative database.
Mortality following hip fracture surgery is higher in patients with dementia than those without; however, few large-scale studies have investigated postoperative in-hospital complications in such patients. The aim of this study was to elucidate the complications that occur after hip fracture surgery in patients with and without dementia using a large national database. ⋯ Our results highlight the need to pay particular attention to surgical site infection, urinary tract infection and respiratory complications in patients with preoperative dementia after hip fracture surgery. These results provide additional useful evidence to inform the management of these patients.

Biomechanical comparison of two surgical techniques for press-fit reconstruction of the posterolateral complex of the knee.
To date, various surgical techniques to treat posterolateral knee instability have been described. Recent studies recommended an anatomical and isometric reconstruction of the posterolateral corner addressing the key structures, such as lateral collateral ligament (LCL), popliteus tendon (POP) and popliteofibular ligament (PFL). Two clinical established autologous respective local reconstruction methods of the posterolateral complex were tested for knot-bone cylinder press-fit fixation to assess efficacy of each reconstruction technique in comparison to the intact knee. ⋯ Both PLC reconstructions showed equal biomechanical stability as the intact posterolateral knee structures when using knot-bone cylinder press-fit fixation. We registered restoration of the rotational and varus-valgus stability with both surgical techniques. The anterior-posterior translational stability was not influenced significantly. The Larson technique showed significant higher varus/valgus stability in 90° flexion. The latter is easier to perform and takes half the preparation time, but needs grafting of the semitendinosus tendon. The Kawano reconstruction technique is an interesting alternative in cases of missing autografts.

Sonographical parameters of the finger pulley system in healthy adults.
To establish normative values of tendon to bone distances (TBDs) to evaluate the A2 and A4 annular pulley integrity, we hypothesized that these values correlate with gender, athletic exercise, occupation, individual's age and body height. ⋯ Establishing normative values will help to detect injured pulleys more precisely and examination should be performed both in resting position and active forced flexion.

Gait characteristics and quality of life perception of patients following tibial plateau fracture.
The purpose of the current study was to evaluate the long-term functional outcome as measured by gait patterns and quality of life assessment of patients with high-energy tibial plateau fracture compared to matched controls. ⋯ Long-term deviations in gait and quality of life exist in patients following tibial plateau fracture. Patients following tibial plateau fracture present altered spatiotemporal gait patterns compared to healthy controls, as well as self-reported quality of life.

Gender differences in the accuracy of joint line tenderness for arthroscopically confirmed meniscal tears.
The reliability of joint line tenderness was previously investigated among other clinical tests for the diagnosis of meniscal pathology with variable results. The aim of this study was to evaluate and compare the accuracy of joint line tenderness as a clinical diagnosing test for arthroscopically confirmed meniscal tears between males and females. ⋯ The physical finding of joint line tenderness of the knee as a test for lateral meniscal tear was found reliable in both males and females. For medial meniscal tears, the test had low reliability and thus less useful if used alone, in both genders.
