Tranexamic acid usage in hip fracture surgery: a meta-analysis and meta-regression analysis of current practice.
The use of tranexamic acid (TXA) in hip fracture surgery remains inconclusive. The aim of the present meta-analysis was to assess the role of TXA use in hip fracture surgery, and attempt to disclose possible factors which might influence TXA efficacy and safety. ⋯ Current evidence indicated that intravenous administration of TXA could significantly reduce blood transfusion and blood loss without increasing risk of thromboembolic events. The frequency and dosage of TXA might not alter the beneficial effect. The application of topical TXA should be cautious.

Multiple platelet-rich plasma injections are superior to single PRP injections or saline in osteoarthritis of the knee: the 2-year results of a randomized, double-blind, placebo-controlled clinical trial.
The primary purposes of this study were to prove the efficacy of PRP injection therapy on knee pain and functions by comparing patients with mild to moderate OA with a placebo control group, and also to understand the effectiveness of multiple doses compared to a single dose. It was hypothesized that PRP would lead to more favorable results than the placebo at 1, 3, 6, 12 and 24 months after treatment. ⋯ NCT04454164 (ClinicalTrials.gov identifier).

Improved femoral component rotation in total knee arthroplasty: an anatomical study with optimized gap balancing.
Surgically balanced total knee arthroplasties have shown improved functional and clinical outcomes. Two different alignment methods have been proposed, the measured resection technique which uses femoral landmarks on the one hand and the ligament balanced technique which uses spreaders on the other. As anatomical landmarks also vary widely, with regards to the tibial cut irrespective of the collateral ligaments, we hypothesized that anatomical landmarks are not suitable for ideal femoral component rotational alignment. ⋯ Based on this study flexion gap balancing can be better optimized using ligament balancing technique. As this technique is highly dependent on the proximal tibial cut, we do recommend the use of navigational devices, which additionally assure a neutral leg alignment.

Review Case ReportsSpontaneous iliopsoas tendon rupture: a case report and brief review.
This report aims to describe the epidemiology, clinical presentation, diagnostic workup, treatment, and rehabilitation, natural course, and possible risk factors of the rare entity spontaneous iliopsoas tendon rupture. ⋯ Spontaneous iliopsoas tendon rupture is a rare event, typically occurring in the elderly female. MRI is generally diagnostic, but signs are also often present on CT scans and sometimes even on plain X-ray. Conservative treatment has an excellent prognosis of full recovery within 6 weeks. The evidence on possible risk factors is scarce and conflicting.

Management of femoral head fractures through surgical hip dislocation (SHD): a demanding but safe technique.
Femoral head fractures considered to be rare injuries. Surgical intervention is indicated for major fragment displacement or in the presence of instability. Surgical management can be achieved through either Anterior, posterior, and trans-trochanteric surgical approaches. Surgical hip dislocation (SHD) has been advocated by many authors to be a safe and effective alternative way of management. The aim of this study was to report on the accuracy of fracture reduction, procedure safety, and outcomes of using SHD in the management of femoral head fractures. ⋯ SHD offers a safe and efficient approach for femoral head fractures management with acceptable clinical outcomes as well as complication rates. Giving the advantage of fully exposing the femoral head and the acetabulum which enables the surgeon to anatomically reduce the fracture and treat any associated injuries, SHD is recommended besides other approaches for the management of femoral head fractures.
