Birth outcomes in pregnant women treated with low-molecular-weight heparin.
Pregnancy and puerperium are associated with an increased risk of venous thromboembolism. Low-molecular-weight heparin is the anticoagulant of choice in pregnant women because, unlike warfarin, it does not cross the placenta. However, there are limited data on the risk of adverse birth outcomes following use of low-molecular-weight heparin in pregnancy. ⋯ We have provided additional evidence of the safety of low-molecular-weight heparin use in pregnancy.

Clinical TrialTermination of second trimester pregnancy with mifepristone and gemeprost. The clinical experience of 197 consecutive cases.
Earlier controlled clinical trials have demonstrated that combined treatment with the antiprogestagen, mifepristone and a suitable prostaglandin reduce the induction to abortion time in second trimester abortion. The aim of this study was to describe the results of the 197 consecutive second trimester terminations performed in routine clinical practice at our Department from 1996 to 1998. ⋯ The study confirms the efficacy and safety of mifepristone, together with gemeprost, for termination of second trimester pregnancy when routinely used in the clinic.

Epidural analgesia with sufentanil during labor and operative delivery.
It has been argued that by adding an opioid to the local anesthetic drug used for epidural analgesia during childbirth, one can reduce the risk of operative delivery. Objective. In a population-based observational study, to evaluate the effect of adding an opioid to a local anesthetic drug on the risk of instrumental delivery or cesarean section. ⋯ When added to the local anesthetic used for epidural analgesia, as in Sweden during the last 5 years, opioids appear to reduce the incidence of instrumental delivery and cesarean section and also the post partum hospital stay.

Randomized Controlled Trial Clinical TrialThe effect of tamoxifen on the endometrium, serum lipids and hypothalamus pituitary axis in the postmenopausal breast cancer patients.
There is still no cost-effective endometrial screening method for asymptomatic postmenopausal breast cancer patients using tamoxifen. We investigated the effectivity of transvaginal ultrasonography and endometrial sampling as a screening method for asymptomatic patients. Additionally the effect of tamoxifen on hypothalamus-pituitary axis and serum lipid profiles were investigated. ⋯ Ultrasonographic imaging of the endometrium in asymptomatic postmenopausal breast cancer patients using tamoxifen should be interpreted with caution. Other imaging techniques should be used for more specific information about the endometrium.

Fetal oxygen saturation during epidural and paracervical analgesia.
We wanted to assess changes in fetal oxygenation during maternal epidural or paracervical analgesia in labor. ⋯ There was a small but significant difference in fetal oxygenation between epidural and paracervical groups during the observation period. The magnitude of the difference is hardly clinically significant. A larger, randomized study is needed to elucidate the mechanisms behind this finding.
