Cumulative sum (CUSUM) analysis in the assessment of trainee competence in fetal biometry measurement.
To determine the value of cumulative sum (CUSUM) analysis in assessing trainee proficiency in fetal biometry measurement. ⋯ CUSUM is a useful tool for identifying points of competence and for quantifying the duration of ultrasound training required for each trainee. It provides an early indication of performance, and highlights difficulties in individual performance.

Sonographic findings of placental lacunae and the prediction of adherent placenta in women with placenta previa totalis and prior Cesarean section.
To investigate the value of transvaginal sonographic findings of intraplacental lacunae for predicting adherent placenta and clinical outcome in patients with placenta previa totalis and a history of Cesarean section. ⋯ Transvaginal sonographic findings of intraplacental lacunae in patients with placenta previa totalis and a history of Cesarean section are useful in the prediction of adherent placenta and may have a role in the prediction of clinical outcome.

Case ReportsDevelopment of hypoplastic left heart syndrome after diagnosis of aortic stenosis in the first trimester by early echocardiography.
We describe a case of aortic stenosis in the first trimester that progressed to hypoplastic left heart syndrome. At 11 + 3 weeks' gestation the four-chamber view was normal but evidence of increased aortic blood velocity prompted the diagnosis of aortic outflow obstruction. At 16 + 6 weeks' gestation the left ventricle was hyperechogenic, globular and dysfunctional, suggesting the development of hypoplastic left heart syndrome. ⋯ Postnatal intervention included a Norwood I operation performed on the 5th postnatal day. We have shown the feasibility and utility of early fetal echocardiography and that a left heart obstruction diagnosed in the first trimester can progress to left heart hypoplasia during the early second trimester. This information is important for planning prenatal and perinatal management.

New models to predict depth of infiltration in endometrial carcinoma based on transvaginal sonography.
Preoperative knowledge of the depth of myometrial infiltration is important in patients with endometrial carcinoma. This study aimed at assessing the value of histopathological parameters obtained from an endometrial biopsy (Pipelle de Cornier; results available preoperatively) and ultrasound measurements obtained after transvaginal sonography with color Doppler imaging in the preoperative prediction of the depth of myometrial invasion, as determined by the final histopathological examination of the hysterectomy specimen (the gold standard). ⋯ Single morphological parameters do not improve the predictive power when compared with the subjective assessment of depth of myometrial invasion of endometrial cancer, and blood flow indices do not contribute to the prediction of stage. In this study an LS-SVM model with an RBF kernel gave the best prediction; while this might be more reliable than subjective assessment, confirmation by larger prospective studies is required.
