Effect of footwear on intramuscular EMG activity of plantar flexor muscles in walking.
One of the purposes of footwear is to assist locomotion, but some footwear types seem to restrict natural foot motion, which may affect the contribution of ankle plantar flexor muscles to propulsion. This study examined the effects of different footwear conditions on the activity of ankle plantar flexors during walking. Ten healthy habitually shod individuals walked overground in shoes, barefoot and in flip-flops while fine-wire electromyography (EMG) activity was recorded from flexor hallucis longus (FHL), soleus (SOL), and medial and lateral gastrocnemius (MG and LG) muscles. ⋯ Although walking with shoes required higher muscle activity for propulsion than walking barefoot or with flip-flops in most participants, this did not result in statistically significant differences in EMG amplitude between footwear conditions in any muscle (p > 0.05). Time to peak activity showed the lowest coefficient of variation in shod walking (3.5, 7.0, 8.0 and 3.4 for FHL, SOL, MG and LG, respectively). Future studies should clarify the sources and consequences of individual EMG responses to different footwear.

Lower limb kinematics in individuals with chronic low back pain during walking.
Several investigators have suggested the presence of a link between Chronic Low Back Pain (CLBP) and lower limbs kinematics that can contribute to functional limitations and disability. Moreover, CLBP has been connected to postural and structural asymmetry. Understanding the movement pattern of lower extremities and its asymmetry during walking can provide a basis for examination and rehabilitation in people with CLBP. ⋯ In the control group, knee moved through a significantly different motion pattern in the transvers plane on the dominant side in comparison with the non-dominant side. In conclusion, low back pain lead to altered movement patterns of the main joints of lower limbs especially on the dominant side during stance phase. Therefore, care should be taken to examine dominant lower limb movement pattern in CLBP to make a better clinical decision.

Respiratory muscle activation and action during voluntary cough in healthy humans.
Cough is a defensive airway reflex consisting of a modified respiratory act which involves the sequential activation of several laryngeal and respiratory muscles. The contraction of the latter results in thoraco-abdominal volume variations in order to provide enough amount of air available, the operating volume (OV), to be expelled. Because both posture and OV could influence muscular activation and thoraco-abdominal displacements during voluntary cough, we aimed to verify if and how they play a role during inspiratory (ICP) and expiratory (ECP) cough phases, in terms of flow, volumes and surface electromyography activity (sEMG). ⋯ Postures slightly affected only sEMG of the thoracic muscles. sEMG data were similar (p > 0.05) in the other OV, but cough peak flow increased with OV. Thoraco-abdominal volume variations during cough were unaffected by posture and OV as well, being predominantly thoracic (supine: 60 and 64%; seated: 68 and 69%, respectively during ICP and ECP). Our results suggest that voluntary cough OV or posture do not have an important effect on voluntary cough that seems more likely to be resulting from a motor mechanism that activates a synergetic antagonistic contraction of inspiratory and expiratory muscles leading to a specific thoraco-abdominal pattern, in which the rib cage is the predominant.

Observational StudyUse of accelerometers for automatic regional chest movement recognition during tidal breathing in healthy subjects.
Recognition of breathing patterns helps clinicians to understand acute and chronic adaptations during exercise and pathological conditions. Wearable technologies combined with a proper data analysis provide a low cost option to monitor chest and abdominal wall movements. Here we set out to determine the feasibility of using accelerometry and machine learning to detect chest-abdominal wall movement patterns during tidal breathing. ⋯ The second cluster included sensors at the fourth right rib midline, right and left seventh ribs, abdomen at linea alba, and right and left tenth thoracic vertebra midline. Costal-superior and costal-abdominal patterns were also recognized. We conclude that accelerometers placed on the chest and abdominal wall permit the identification of two clusters of movements regarding respiration biomechanics.

Comparative StudyCompensatory strategy between trunk-hip kinematics and reaction time following slip perturbation between subjects with and without chronic low back pain.
Compensatory trunk and hip motions following slip perturbations may compromise the control of lumbopelvic movement. However, it is unclear how postural integration of the trunk and hips can be transferred to treadmill-induced slip perturbations in subjects with chronic low back pain (LBP). The purpose of this study was to investigate trunk reaction times and three-dimensional trunk-hip angle changes following a slip perturbation (duration: 0.12 sec, velocity: 1.37 m/sec, displacement: 8.22 cm) with a handheld task between subjects with and without chronic LBP. ⋯ Clinicians need to consider compensatory strategies to improve trunk flexibility following slip perturbations in subjects with chronic LBP. Mini abstract: Trunk reaction time and three-dimensional trunk-hip motions were compared between subjects with and without chronic low back pain (LBP). The control group demonstrated greater trunk flexion; however, the LBP group reduced trunk flexion to protect against further injuries following the novelty of the slip perturbation.
