Vascular malformations are infrequent causes of aqueductal stenoses, developmental venous anomaly (DVA) being the rarest among them. DVAs, also known as venous angiomas, are congenital in origin and characterized by dilatation of vessels in the superficial and deep venous system. Although they are usually clinically silent, they can be complicated by hemorrhage, seizures and neurologic deficits. Herein, we report MR imaging findings of a 7-year-old girl whose hydrocephalus was due to an abnormal vein coursing through the aqueduct.

This review seeks to clarify the most important implications of higher magnetic field strength for clinical examinations of the whole body. An overview is provided on the resulting advantages and disadvantages for anatomical, functional and biochemical magnetic resonance examinations in different regions of the body. It is demonstrated that susceptibility-dependent imaging, chemical shift selective (e.g., fat-suppressed) imaging, and spectroscopic techniques clearly gain from higher field strength. ⋯ Currently available and potential future strategies to overcome related limitations are discussed. Whole-body MRI at higher field strength currently leads to clearly improved image quality using a variety of established sequence types and for examination of many body regions. But some major problems at higher field strength have to be solved before high-field magnetic resonance systems can really replace the well-established and technically developed magnetic resonance systems operating at 1.5 T for each clinical application.

Comparative Study
Value of high-resolution ultrasound in detecting a pneumothorax.
This study was designed to compare the detectability of high-resolution ultrasound (HRUS) and bedside chest radiography (CR) for a pneumothorax. During the last 14 months, 97 consecutive patients who were admitted to our institute to undergo a transthoracic needle aspiration and biopsy (TNAB) of the lung were included. Both HRUS and CR were performed immediately after the TNAB procedure. ⋯ The diagnostic accuracies were 89 and 77%. The inter-observer agreement was excellent (kappa=0.85) in the HRUS images and moderate (kappa=0.49) in the CR. The results of this study suggest that HRUS is a more sensitive and confident method for diagnosing a pneumothorax when compared to bedside chest radiography.

Despite the development of many effective antibiotic therapies and the general improvement in hygiene and health care systems all over the world, the incidence of central nervous system (CNS) infection has increased significantly in the past 15 years. This can be attributed primarily to the acquired immunodeficiency syndrome (AIDS) epidemic and its devastating effect on the immune system and secondarily to various immunosuppressive agents that are being used in aggressive cancer treatment and in organ transplantations. ⋯ The early detection and specific diagnosis of infection are of great importance, since brain infections are potentially treatable diseases. Imaging studies play a crucial role in the diagnostic process, along with the history (exposure to infectious agents), host factors (open head trauma, CSF leak, sinusitis, otitis, immune status), physical examination and laboratory analysis of CSF.

The aim of this study was to compare the diagnostic performance of gadobenate dimeglumine (Gd-BOPTA)-enhanced MR imaging, including dynamic phases and one-hour delayed phase, versus superparamagnetic iron oxide (SPIO)-enhanced imaging for detection of liver metastases. Twenty-three patients with 59 liver metastases underwent Gd-BOPTA-enhanced MR imaging (unenhanced, arterial, portal, equilibrium and one-hour delayed phase) using three-dimensional volumetric interpolated imaging and SPIO-enhanced T2-weighted turbo spin-echo and T2*-weighted gradient-echo sequences on a 1.5-T unit. Three observers independently interpreted the three sets of images, i.e. ⋯ In addition, Az values and sensitivities of both imaging sets were significantly higher than those of Gd-BOPTA-enhanced dynamic images (0.826, 77.4%: p<0.05). There was no significant difference in the positive predictive value among the three image sets. Gd-BOPTA-enhanced delayed phase imaging showed comparable diagnostic performance to SPIO-enhanced imaging for the detection of liver metastases, and had a better diagnostic performance than Gd-BOPTA-enhanced dynamic images.
