Comparative StudyNovel method to treat sterno-clavicular joint instability and medial clavicle fracture symptomatic nonunion.
Stabilization of the medial clavicle is traditionally associated with relatively high complication and failure rate. This is a retrospective, case series study to assess outcome of a novel two-stage surgical treatment of sterno-clavicular joint instability or symptomatic medial clavicle fracture non-union. ⋯ We cautiously recommend this method to experienced surgeons. A larger number of patients is required to endorse wider generalization of the results.

The impacted varus (A2.2) proximal humeral fracture in elderly patients: is minimal fixation justified? A case control study.
The purpose of this retrospective case control study was to assess the outcome of K-wire osteosynthesis of a varus displaced proximal humeral fracture in patients over 65 years old, compared to a control group treated nonoperatively. ⋯ Level 3; Case control study, treatment study.

Randomized Controlled Trial Comparative StudyInterscalene versus subacromial continuous infusion of ropivacaine after arthroscopic acromioplasty: a randomized controlled trial.
Interscalene and subacromial infusion of local anesthetics have both been shown to be effective in alleviating pain after shoulder arthroscopy. We performed a prospective randomized clinical trial in which both methods were compared in patients after acromioplasty. ⋯ A continuous subacromial infusion of 0.2% ropivacaine (2 mL/h) is inferior to continuous interscalene infusion, particularly during the first 12 hours, but could be an alternative in patients with contraindications of interscalene anesthesia or when acromioplasty is performed as an outpatient procedure.

Comparative StudyStatic posterior humeral head subluxation and total shoulder arthroplasty.
Static posterior subluxation of the humeral head (PSH) is often associated with glenohumeral arthritis. It may persist following total shoulder arthroplasty (TSA) and lead to accelerated polyethylene wear and glenoid component loosening. The factors which lead to PSH are poorly understood. The purpose of this study was to test the hypothesis that operative correction of glenoid version during shoulder arthroplasty re-centers the glenohumeral joint; therefore, glenoid replacement may be considered even in cases of osteoarthritis associated with posterior humeral head subluxation. ⋯ Level 4; Case series, treatment study.

Comparative StudyA new nerve block procedure for the suprascapular nerve based on a cadaveric study.
Shoulder pain after rotator cuff repair surgery is sometimes very severe. Suprascapular nerve block (SSNB) is a method used to relieve this shoulder pain, but is not always completely effective. The purpose of this study is to develop the new effective suprascapular nerve block procedure. ⋯ Basic science study.
