Randomized Controlled Trial Clinical TrialAnaesthesia with sevoflurane in children: nitrous oxide does not increase postoperative vomiting.
Nitrous oxide (N2O) has been associated with postoperative nausea and vomiting (PONV), but some studies show conflicting data. The aim of this study was to assess whether the combination of N2O/sevoflurane, in paediatric general anaesthesia, increases the incidence of vomiting in the 24 h following surgery compared with sevoflurane alone. ⋯ Nitrous oxide used in combination with sevoflurane is not associated with an increase in the incidence of emesis in children who undergo testicle and inguinal hernia procedures.

The definition of childhood obesity has not been standardized in the past, making studies difficult to compare. In spite of this, the increase in the incidence of childhood obesity is evident and has now reached epidemic proportions. Obese children experience few of the medical complications seen in obese adults. ⋯ There is less pathology in the obese child when compared with the adult but some evidence shows a higher likelihood of a critical incident occurring when anaesthetizing such children. This shows that we need to be as worried about anaesthetizing the obese child as we are for the obese adult. This concern should increase with increasing body mass index.

Randomized Controlled Trial Clinical TrialEffect of route of buprenorphine on recovery and postoperative analgesic requirement in paediatric patients.
We compared the effects of extradural with intravenous (i.v.) buprenorphine on postoperative pain and recovery characteristics. ⋯ Administration of buprenorphine resulted in a higher incidence of side-effects.

Risk factors for airway complications during general anaesthesia in paediatric patients.
Although airway complications are a frequent problem during paediatric anaesthesia, no study has prospectively identified risk factors for adverse respiratory events during airway management when LMA trade mark (laryngeal mask airway), face mask (FM) or a tracheal tube (TT) are used. ⋯ Age < 6 years, recent RI and the use of the LMA were identified as independent factors associated with an increased risk of airway complications.

Case ReportsNiemann-Pick disease: a rare problem in anaesthesiological practice.
We describe the problems encountered during general anaesthesia in a 2-year-old boy who was suffering from an extremely severe form of Niemann-Pick disease. Attention focused on respiratory difficulties and intubation problems. The usefulness of the lateral position and the limits of the Mallampati score in this case are discussed.
