Randomized Controlled Trial Comparative StudyRandomized, controlled trial of multimodal shoulder injection or intravenous patient-controlled analgesia after arthroscopic rotator cuff repair.
The aim of this study was to compare the effect of postoperative pain control and adverse effects of intravenous patient-controlled analgesia (IV PCA) and multimodal shoulder injection after arthroscopic rotator cuff repair. ⋯ Multimodal shoulder injection is a safe and effective modality for management of pain after arthroscopic rotator cuff repair. Considering the expense and need of special devices for IV PCA, multimodal shoulder injection may be an effective and safe alternative to IV PCA for postoperative analgesia after arthroscopic rotator cuff repair.

Excellent results of lesser tuberosity transfer in acute locked posterior shoulder dislocation.
The aim of the present study was to evaluate the clinical and radiological results of lesser tuberosity transfer in acute locked posterior shoulder dislocation with a humeral head defect between 25 and 45 %. ⋯ Lesser tuberosity transfer shows excellent clinical and radiographic mid-term results in acute cases of locked posterior shoulder dislocation with a reverse Hill-Sachs lesion between 25 and 45 %.

Randomized Controlled TrialA randomized double-blind clinical trial of tourniquet application strategies for total knee arthroplasty.
The optimal timing of pneumatic lower limb tourniquet application during primary elective total knee arthroplasty surgery (TKA) is a matter of debate. Most previous reports have failed to show significant differences between different tourniquet timings. The aim of the work was to determine how three strategies of lower limb pneumatic tourniquet application affect the outcome for TKA patients. ⋯ Inflation of an automatic pneumatic lower limb tourniquet before skin incision and its deflation after hardening of cement tends to give better outcomes in TKA patients during six postoperative days. The estimated blood loss was highest when the tourniquet was inflated just before cement application and deflated after its hardening.

Randomized Controlled TrialLocal infusion analgesia using intra-articular double lumen catheter after total knee arthroplasty: a double blinded randomized control study.
Although intra-operative local infiltration analgesia has gained increasing popularity in joint replacement surgery, it is not clear whether postoperative local infusion analgesia using an indwelling catheter provides clinically important additional effects. We, therefore, conducted a randomized controlled trial to clarify the efficacy of the originally developed local infusion analgesia technique in total knee arthroplasty. ⋯ Although larger studies are needed to examine its safety, the local infusion analgesia alone provided clinically significant analgesic effects and rapid recovery in total knee arthroplasty.

The adductor tubercle: a reliable landmark for analysing the level of the femorotibial joint line.
The ratio of femoral width and distance from medial epicondyle to joint line helps estimate the femoral joint line position from femoral width. The approximately radial position of the medial epicondyle on femoral condyle spheres is probably responsible for this relationship, The adductor tubercle approximately lies diametrically opposite to the joint line on condylar sphere. Then, a linear correlation could also exist between the femoral width and distance of adductor tubercle to joint line and is the purpose of the current study. ⋯ We conclude that adductor tubercle can be used as a morphologic landmark to determine the knee joint line position, because a linear correlation between femoral width and distance from the adductor tubercle to the joint line was found.
