A basic arthroscopy course based on motor skill training.
Aim of this study is to introduce, evaluate, and propose a basic arthroscopy training course with interactive methods as using bovine knees, joint models but mainly focusing on practices with motor skill-learning devices. This study presents the results of a skill training program which is based on "motor skill training" theory and using "bovine knees" to simulate a specific task as arthroscopy both in education and evaluation. The participants gave feedback regarding their expectations, fulfillment, and self-evaluation on the degree of their improvement levels, after which their skills were evaluated by an expert while they performed the arthroscopic procedure. ⋯ Pre- and post-course self-ratings of the participants inquiring about their ability to perform an arthroscopy alone showed significant improvement (4.2 vs. 7.7 P = 0.000, paired samples t test). All participants were found to be competent in the evaluation of their diagnostic arthroscopy skills on bovine knees. To design a skill teaching course based on the needs of the trainees, focusing on basic motor skill training exercises, and using bovine knee as a simulator is a safe, inexpensive, humanistic, and replicable method that proves a foundation for basic arthroscopic skills learning prior to patient encounter.

Three different tumour-like lesions within the infrapatellar fat pad, an osteochondroma, a localised pigmented villonodular synovitis and a synovial cyst are reported. The osteochondroma and the pigmented villonodular synovitis were treated by marginal excision, and the synovial cyst was resected using arthroscopy.

Sartorial branch of the saphenous nerve in relation to a medial knee ligament repair or reconstruction.
Surgical approaches to repair or reconstruct the medial knee structures note caution to avoid the sartorial branch of saphenous nerve. However, the approximate area of potential iatrogenic nerve injury has not been previously described in relation to landmarks for a medial knee reconstruction. The purpose of this study was to define the course of the sartorial branch of the saphenous nerve in relation to the superficial medial collateral ligament. ⋯ The distance from the anterior border of the superficial medial collateral ligament to the sartorial branch of the saphenous nerve decreased as the distance was increased distally with a mean distance of 4 cm (4.1 +/- 0.8 cm) distal from the joint line and 6 cm (3.8 +/- 0.8 cm) distal from the joint line. We have characterized the surgically relevant landmark anatomy of the sartorial branch of the saphenous nerve in regards to performing a repair or reconstruction of the medial knee structures. Familiarity with these anatomic landmarks and associated distances from the sartorial branch of the saphenous nerve, we can assess the potential area of vulnerability to this nerve branch intraoperatively.

Randomized Controlled Trial Multicenter StudyAutologous patellar tendon and quadrupled hamstring grafts in anterior cruciate ligament reconstruction: a prospective randomized multicenter review of different fixation methods.
ACL reconstruction with bone patellar tendon bone (BPTB) grafts has been shown to produce dependable results. Recently, reconstructions with double-looped semitendinosus gracilis (DLSG) grafts have become common. The prevailing opinion is that ACL reconstruction with patellar tendon graft produces a more stable knee with more anterior knee pain than DLSG grafts, while the functional results and knee scores are similar. ⋯ BPTB ACL reconstruction fixed with interference screws and DLSG fixed with Bone Mulch Screws on the femur and WasherLoc Screws on the tibia produce satisfactory and nearly identical outcomes. Among our patients in the DLSG group, flexion strength was lower, and more patients underwent meniscus surgery in the follow-up period. The BPTB group has more anterior knee pain.
