Pylephlebitis or septic thrombophlebitis of the portomesenteric veins is a complication of intra-abdominal infections. The disease is rare in children and the diagnosis is often delayed. The morbidity of pylephlebitis is relatively low, although there is a risk of residual thrombosis. ⋯ An appendectomy is always performed either at the time of diagnosis or later. The need for anticoagulation therapy in children is controversial. However, most pediatricians recommend its use, beginning as soon as possible, to be continued until normalization of portal vein flow.

Sickle cell anemia is the most common hemoglobinopathy worldwide; its musculoskeletal complications consist more often of medullary infarctions involving long bones. Orbital infarction is uncommon. We report on a case of orbital subperiosteal hematoma in a 9-year-old male with a medical history of sickle cell thalassemia.

In France, recent legislation for pediatric critical care organization endorsed the creation of a new level of pediatric care, the intermediate care units. These units treat children who require close monitoring and/or continuous monitoring due to potential failure of 1 or more organs, yet not to the degree of intensity of pediatric critical care. These rules do not provide precise admission and discharge guidelines for the pediatric patients requiring intermediate care. ⋯ We also described the diseases and potential organ failures of the patients who were referred to these intermediate care units. This study provides an estimate of the demand for pediatric intermediate care as defined in the French legislation and therefore helps implement projects to create such units. More specific criteria are still needed to oversee the implementation of these projects.

Tracheobronchial rupture after blunt trauma is rare, especially in a pediatric population. In this paper, we report the case of a 3-year-old child who presented with a rupture of the tracheobronchial tree as a result of multiple injuries (thoracic and cerebral) sustained from a traffic accident. ⋯ As tracheobronchial rupture is a rare condition, particularly in children, physicians must have a high index of suspicion. The diagnosis and treatment of this condition are discussed.
