Acute mediastinitis is uncommon. When it occurs, it usually follows an esophageal perforation or thoracic surgery. ⋯ We report on a case of a ten-year-old girl with non traumatic mediastinitis secondary to a pleuro-pulmonary infection due to Klebsiella pneumoniae.

According to several recent surveys, around 50% of the deaths occurring nowadays in French neonatal intensive care units result from a medical decision. This has led French neonatologists to set up guidelines for end-of-life decisions and practice in the perinatal period, which are presented in this paper. It covers definitions, clinical situations, ethical principles, obligations of the medical and nursing staff.

Early interventions, such as occlusive wrapping of very low birth weight infants at delivery reduce postnatal temperature fall. This new intervention was implemented in our hospital on January 2000. The aim of this study was to investigate retrospectively the effect of polyethylene wrap, applied immediately at birth, on thermoregulation. ⋯ Occlusive wrapping with a polyethylene bag at birth prevented low rectal temperature in premature infants in the immediate postnatal period. This method is easy, practical and effective, and does not interfere with current practice for resuscitation.

[Extracorporeal oxygenation in newborn infant born to diabetic mother with severe hypertrophic myocardiopathy].
Hypertrophic cardiomyopathy is a common complication in infants born to diabetic mothers but it is rarely life threatening. ⋯ Extracorporeal membrane oxygenation can be an efficient transitory therapy for very severe hypertrophic cardiomyopathy in infants of diabetic mothers.
