When the patient condition contraindicates major surgery for descending thoracic aneurysms, the surgeon should consider using an ascending aorta to abdominal aorta bypass graft, leaving the diseased segment undisturbed. Our experience with eight patients is presented. ⋯ In complex aortic coarctation, extra-anatomic bypass operation remains an effective procedure. The usage of these procedures in patients with descending aortic aneurysms remains complicated.

Relevance of platelet transfusion is related to an increasing number of indications due to conditions with an increased risk for hemorrhage and a lack of standard protocols both for treatment and prophylaxis of thrombocytopenia, in the presence of high costs of this procedure. The appropriate use of platelet transfusion is associated with reduction of thrombocytopenia, which actually in a critically ill patient is associated with increased length of stay both in intensive care unit and hospital. In 2003, the Professional Consensus was established to define the main recommendations for platelet transfusion. ⋯ It is recommended that the platelet count in one dose of platelets transfused would be not lower than 55 x 10(9)/L in order to increase the platelet count by 5-10 x 10(9)/L. The effectiveness of platelet transfusion is evaluated 10-60 min, 18 hours, and 24 hours after transfusion. Advantages of platelet transfusion of small and large doses are presented.

[Relevance of examination and treatment of the most common gastrointestinal disorders in children in Lithuania during the last decade].
In the last decade, scientific studies in the field of children's gastroenterology performed in Lithuania explored different problems: pathology of Helicobacter pylori infection and food allergy. Our studies revealed that children with atopic dermatitis had gastrointestinal complaints (abdominal pain, diarrhea, distension and unstable stool, which appeared with the exacerbation of skin rash) more often as compared to nonallergic children of the control group. Abdominal pain in children with atopic dermatitis with local rash was more frequent and lasted longer than in control group children, whereas children with extended rash had stools more frequently. ⋯ The highest frequency of H. pylori infection was found in children with duodenal ulcer; histological changes in their gastric pylorus and corpus mucosa were greatest. More than half of children with nonulcer dyspepsia were infected with H. pylori. After eradication of H. pylori infection, the prevalence of dyspepsia in children with duodenal ulcer decreased.

Gunshot injuries to the paranasal sinuses and orbita are uncommon. Their severity depends on missile track in tissues. ⋯ Functional endoscopic sinus surgery is the most appropriate technique for removing projectiles left. This article also presents one case of airgun injury to the sphenoid sinus with retained missile that was safely removed using endoscopic procedures.

The aim of this study was to determine the impact of the impairment of the body functions on the participation of people with disabilities in sports activities. ⋯ The syndrome of movement function impairment, duration of impairment, marital status, the age of persons with disabilities, and objectively determined impairment of biosocial self-service functions did not impact sports participation of people with disabilities. However, subjective sensation of pain and the need of medical aid, which did not match the objective functional impairments, could be the obstacle for people with disabilities to participate in sports.
