Although the patellofemoral (PF) joint shape of the femoral implant in a particular prosthetic design is identical, the articular surface shape of the patella varies between individuals. The present study investigated the effects of patellar shape on the postoperative PF joint in a cruciate-retaining total knee arthroplasty (CR-TKA) without patellar resurfacing and with at least three years of follow-up. ⋯ This study found that for a patella with a small patellar facet angle, lateral tilt of the patella was significantly increased after TKA, and a high rate of osteosclerosis was evident at >3years after operation. Performing TKA without patellar resurfacing on a patella with a small patellar facet angle may entail a higher risk of postoperative AKP; pre-operative evaluation of the shape of the patella is therefore important.

Periprosthetic joint infection is a serious complication and reconstruction after failed revision total knee arthroplasty with significant bone loss and compromised soft-tissues can be challenging. Objective of this study was to assess clinical and functional results, implant survival and infection recurrence rates in patients treated with two-stage arthrodesis after failed revision TKA with extensor mechanism deficiencies due to PJI, and to identify the factors that affect outcomes after surgery. ⋯ Septic failure of revision knee arthroplasty can be effectively treated with two-stage arthrodesis using a modular intramedullary nail, providing a stable and painless limb with satisfactory functional results and acceptable infection eradication rates.

Anterior cruciate ligament (ACL) graft failure is a complication of ACL reconstruction (ALCR). Smaller autograft diameter may be a contributing factor. Our aim was to determine if hamstring autograft diameter influences graft rupture and patient-reported outcome scores. ⋯ Level IV, retrospective case series.

Aseptic loosening, osteolysis, and infection are the most commonly reported reasons for revision total knee arthroplasty (TKA). This study examined the role of implant design features (e.g. condylar box, pegs) and stems in resisting loosening, and also explored the sensitivity of the implants to a loose surgical fit due to saw blade oscillation. ⋯ In conclusion, long stemmed prostheses appeared less susceptible to surgical cut errors than short stemmed and stemless implants.

Total knee arthroplasty (TKA) significantly improves pain and restores a considerable degree of function. However, improvements are needed to increase patient satisfaction and restore kinematics to allow more physically demanding activities that active patients consider important. The aim of our study was to compare the alignment and motion of kinematically and mechanically aligned TKAs. ⋯ Kinematic alignment resulted in greater variation than mechanical alignment for all tibio-femoral and patello-femoral motion. Kinematic alignment places TKA components patient specific alignment which depends on the preoperative state of the knee resulting in greater variation in kinematics. The use of computational models has the potential to predict which alignment based on native alignment, kinematic or mechanical, could improve knee function for patient's undergoing TKA.
