The purpose of this study was to evaluate the frequency of combined ipsilateral talar and calcaneal fractures, the fracture pattern in combination and the clinical outcome of the combined operative treatment. ⋯ Combined ipsilateral talar and calcaneal fractures are rare injuries. Extraarticular calcaneal fractures are more common in this injury pattern, while there was no preference for either talar neck or talar body fractures. Operative treatment with anatomic open reduction and internal fixation may result in favorable clinical outcome. Combined surgical approaches to the hindfoot did not result in increased morbidity.

There is minimal experience with less rigid syndesmotic fixation devices which may approximate the normal distal tibio-fibular mechanics during healing. This study evaluates the ability of a FiberWire-button implant (Arthrex, Naples, FL) to maintain syndesmotic reduction as compared with a metallic screw. ⋯ The FiberWire-button implant may not maintain adequate ankle syndesmotic reduction in the immediate post-operative period relative to a metallic screw.

We describe a rare ankle fracture pattern called "the Bosworth fracture," originally described in 1947. A Bosworth fracture has been defined as a fixed posterior fracture-dislocation of the distal part of the fibula in which the proximal fibular shaft fragment locks behind posterior tibial tubercle. ⋯ We describe a new radiographic sign seen on anterior-posterior mortise radiographic view showing cortical density in the axilla of the medial tibial plafond: "the Axilla sign." This feature was also noted on the radiographs of other case reports with this unique injury. The presence of this sign should alert the treating surgeon to a possible Bosworth fracture.
