PercuTwist tracheostomy in an intubated patient with tracheal stent.
The optimal placement of a tracheal tube following percutaneous dilator tracheostomy is technically demanding in patients with tracheal stents. We report a successful technique of tracheostomy tube placement with PercuTwist in an intubated patient with a tracheal stent for malignant stenosis. The endotracheal tube was replaced with a laryngeal mask, a 9.0-mm tracheostomy tube was inserted into the trachea over a PercuTwist percutaneous dilator and through the tracheal stent under video-bronchoscopic vision. The PercuTwist technique kept the lumen of trachea, a laryngeal mask enabled adequate ventilation, and a video-bronchoscope guided the prompt insertion of the tracheostomy tube through the stent.

Case ReportsNew-onset cardiac mass due to left atrial appendage inversion during cardiac surgery.
Cardiac mass assessment is challenging in the intraoperative scenario. We present the case of a 22-year-old woman with a new-onset left atrial mass during cardiopulmonary bypass following closure of a ventricular septal defect. We discuss the role of intraoperative echocardiographic examination, the differential diagnosis, and raise awareness of the left atrial appendage as a rare etiology of an acute left atrial mass.

United in earnest: First pilot sites for increased surgical capacity for rheumatic heart disease announced by Cardiac Surgery Intersociety Alliance.
Rheumatic heart disease affects more than 33,000,000 individuals, mostly from low- and middle-income countries. The Cape Town Declaration on Access to Cardiac Surgery in the Developing World was published in August 2018, signaling the commitment of the global cardiac surgery and cardiology communities to improving care for rheumatic heart disease patients. ⋯ Substantial progress has been made since the passing of the Cape Town Declaration and the formation of the Cardiac Surgery Intersociety Alliance, but ongoing efforts with collaboration of all committed parties-cardiac surgery, cardiology, industry, and government-will be necessary to improve access to life-saving cardiac surgery for rheumatic heart disease patients.

Case ReportsOrganization of extracorporeal membrane oxygenation services for COVID-19.
Extracorporeal membrane oxygenation has been used for COVID-19 patients with refractory hypoxemia. ⋯ Extracorporeal membrane oxygenation is an option for COVID-19 patients but preparation must be taken to prepare the extracorporeal membrane oxygenation teams to deal with this pandemic and future challenges.

Surgical repair of anomalous aortic origin of coronary artery in adults.
Anomalous aortic origin of a coronary artery is a congenital abnormality of the origin or course of a coronary artery that arises from the aorta. The surgical treatment of this anomaly is highly variable and controversial and is achieved by implementing elaborate techniques of anatomic repair or by simple coronary artery bypass grafting. This review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. ⋯ Anatomic repair is technically demanding and should be carried out in experienced centers. Coronary artery bypass with internal thoracic artery or vein grafts is technically straightforward with low operative risk and comparable midterm outcomes, however, long-term outcomes are unknown. Coronary artery bypass grafting is the technique of choice for older patients, in those with concomitant coronary artery disease, as a bailout procedure for failed anatomic repair, or in centers without experience in anatomic repair for anomalous aortic origin of a coronary artery.
