Long-term results of aortic valve replacement with posterior root enlargement.
Posterior root enlargement provides the implantation of suitable-sized prosthetic valves in patients with a small aortic root. The aim of this study was to evaluate the long-term results of posterior root enlargement in patients undergoing aortic valve replacement. ⋯ Aortic root enlargement using the Manouguian technique can be applied with acceptable safety. It effectively decreases postoperative gradients and increases left ventricular ejection fraction. Short- and long-term mortality and morbidity rates are satisfactory.

Intraoperative tranexamic acid in pediatric bloodless cardiac surgery.
The aim of this study was to evaluate the clinical effects of intraoperative tranexamic acid administration in cardiac surgery without blood transfusion (bloodless cardiac surgery) in children. ⋯ Intraoperative tranexamic acid administration was effective for blood conservation, and improved postoperative clinical outcomes in pediatric bloodless cardiac surgery.

Video-assisted thoracoscopic surgery for nontuberculous mycobacterial infection.
The number of patients with nontuberculous mycobacterium infection is increasing in Japan, and therefore surgical treatment is also being applied with increasing frequency. Although the effectiveness of surgery for nontuberculous mycobacterium disease has been established and reported by a number of authors, the role of video-assisted thoracoscopic surgery in the surgical treatment of nontuberculous mycobacterium disease has not been sufficiently investigated. ⋯ Video-assisted thoracoscopic surgery is a safe and useful procedure for lobectomy and segmentectomy in patients with nontuberculous mycobacterium disease.

Virchow's triad and intestinal ischemia post cardiac surgery.
Intestinal ischemia is associated with a very high mortality rate. We combined the principles of Virchow's triad to produce preoperative and postoperative models for the development of intestinal ischemia. ⋯ Potentially remedial causes of intestinal ischemia include blood product usage, reoperation for bleeding, and creatine kinase-MB release. Age, female sex, peripheral vascular disease, intraaortic balloon pump usage, and preexisting renal failure are fixed risk factors. Despite the continuing trend of reduced blood product usage in the field of cardiac surgery, the increase in patients' risk factors will mean that incidences of intestinal ischemia may increase in the future.

Case ReportsObstructive bioprosthetic mitral valve thrombus: management options?
Bioprosthetic valve thrombosis is an extremely rare event, therefore, long-term anticoagulation can be avoided. There is limited experience in the diagnosis and treatment of such a situation. We present the case of a patient with a porcine mitral bioprosthesis who presented with acute pulmonary edema, likely secondary to obstructive valve thrombosis. A favorable outcome was observed after conservative anticoagulant treatment.
