Review Case ReportsIntrathecal vincristine: an analysis of reasons for recurrent fatal chemotherapeutic error with recommendations for prevention.
Accidental intrathecal vincristine instillation is usually a fatal error. The authors report an analysis of a patient and suggest means with which to reduce such errors. ⋯ Intrathecal injection of vincristine may be the end-result of a series of systems errors. Protocol recommendations to reduce the likelihood of this error are presented.

Review Case ReportsSpinal canal involvement in infantile myofibromatosis: case report and review of the literature.
An unusual case of infantile myofibromatosis with spinal canal involvement is reported and the literature is reviewed. ⋯ Only six cases of infantile myofibromatosis associated with spinal canal involvement have been reported. Three patients with flaccid paresis of extremities and respiratory distress died in the newborn period. The other three patients showed improvement of the paresis. The prognosis of infantile myofibromatosis without visceral complication is generally good, but neurologic impairment may occur at birth if the spinal cord is compressed.

Septicemia and septic shock in pediatric patients: 140 consecutive cases on a pediatric hematology-oncology service.
This report describes the incidence of septic shock in pediatric hematology-oncology patients with positive blood cultures and investigates parameters of potential use in early diagnosis of gram-negative (GN) bacteremia and septic shock. ⋯ GN organisms were the major cause of septic shock in a group of pediatric hematology-oncology patients with positive blood cultures although they were recovered less frequently than gram-positive organisms. In our study, non-neutropenic patients with indwelling catheters were at approximately the same risk for GN shock as neutropenic patients. Monitoring blood carbon dioxide content may be useful in the early diagnosis of GN infection.

In vitro antitumor activity of rebeccamycin analog (NSC# 655649) against pediatric solid tumors.
This study determined the in vitro antitumor activity of a rebeccamycin analog (NSC# 655649) using tetrazolium/formazan (MTT) and clonogenic assays against established pediatric cell lines and solid tumor specimens obtained from children. ⋯ NSC# 655649 appears to have broad antitumor activity in vitro against pediatric malignancies at drug concentrations achieved during adult phase I clinical trials. These studies support the further development of NSC# 655649 for solid tumors in children.

Case ReportsPhenytoin-induced thrombocytopenia treated with intravenous immune globulin.
To describe the use of intravenous immune globulin (IVIG) in an 8-year-old girl with phenytoin-induced thrombocytopenia and leukopenia. ⋯ Phenytoin hypersensitivity can present with thrombocytopenia and leukopenia. Treatment with IVIG was associated with a rapid rise in the platelet count in the patient in this report. IVIG should be considered for patients with phenytoin-induced thrombocytopenia.
