To investigate the impact of including private-sector data on assessments of equity of coronary revascularisation provision using NHS data only. ⋯ Private provision exacerbates geographical inequalities. Those responsible for commissioning care for defined populations must have access to consistent data on provision of treatment wherever it takes place.

To compare implications of using the logistic EuroSCORE and a locally derived model when analysing individual surgeon mortality outcomes. ⋯ The uncalibrated EuroSCORE significantly overpredicted mortality and is not recommended. Instead, the EuroSCORE should be calibrated for contemporary performance. The differences demonstrated in defining high mortality outcomes when using a model built for purpose suggests that the choice of risk model is important when analysing surgeon mortality outcomes.

To examine arterial and left ventricular function and their interaction in patients with early-stage chronic kidney disease (CKD). ⋯ Early-stage CKD is characterised by reduced aortic distensibility and increases in arterial, ventricular systolic and diastolic stiffness; arterial-ventricular coupling is preserved. This pattern of pathophysiological abnormalities resembles that seen in heart failure with preserved ejection fraction and may account for the high levels of cardiovascular morbidity and mortality in patients at all stages of CKD.
