Case ReportsUrinary Tract Infection in a Renal Transplant Patient by Myroides Species: A Case Report from Pakistan.
Myroides species are mostly considered to have low pathogenic potential causing infections in immuno-compromised patients; though, few cases of urinary tract infections (UTIs), bacteremia, pneumonia, endocarditis, ventriculitis, skin, and soft tissue infections have been reported. The two most common species associated with human infections are M. odoratus and M. odoratimimus. Due to the multi-drug resistant nature of this bacteria, prompt identification and antibiotic susceptibility testing in the laboratory are of paramount importance in order to direct the clinician towards an appropriate antibiotic treatment. ⋯ This is the first reported case of a clinically significant infection caused by Myroides species from Pakistan. The patient in this case responded favorably to treatment by oral minocycline and was treated successfully. Key Words: Myroides, Multi-drug resistant, Urinary tract infection, Renal transplant.

Case ReportsMalignant Phyllodes Tumour with Osteoclast-type Giant Cells in Young Females: A Rare Histology and Differential Diagnoses.
Phyllodes tumour (PT) is a biphasic fibroepithelial tumour of the breast that accounts for 0.3-1% of all primary breast neoplasms. It is subdivided into benign, borderline, and malignant categories based on histologic features including stromal overgrowth, stromal hypercellularity, mitotic count, degree of atypia, and type of margins. Malignant PT (MPT) is characterised by infiltrative margins, marked stromal overgrowth, increased stromal hypercellularity, greater than 10 mitoses/10 high-power fields (HPFs), and moderate to severe atypia. ⋯ We herein report cases of MPT with OGCs in two young females who had a history of swelling in unilateral breast for two-three years. Histologic examination revealed classic phyllodes areas admixed with a sarcomatous stromal component with OGCs. Key Words: Malignant phyllodes tumour, Breast, Osteoclast-type giant cells.

Case ReportsMultiple Brain Abscesses and Mycotic Aneurysm in a Patient with Lupus Nephritis: A Challenging Diagnosis and Treatment Course.
Central nervous system (CNS) septic embolisation is an uncommon but life-threatening condition. Clinicians should be aware of the increased risk of this complication in immunocompromised patients such as those suffering from lupus nephritis (LN). In cases of unexplained fever or neurologic symptoms, septic embolisation should be considered in the differential diagnosis. ⋯ Blood, cerebrospinal fluid, and bronchoalveolar lavage cultures were negative. The patient was treated with a combination of antibiotics and she responded well to treatment. Key Words: Female, Lupus nephritis, Mycotic aneurysm, Brain abscess.

Differentiating between benign cutaneous manifestations and lesions from inflicted injuries is not always simple. A 14-year boy was admitted to our Forensic Medicine Clinic for the preparation of a medico-legal report requested by the prosecution office. On inspection, there were 10 violaceous, atrophic, horizontal, linear striae distensae (SD), confined to the lower back and measuring 0.5 cm in width and between 6 cm and 13 cm in length. ⋯ It is recommended that practitioners develop examination skills and medical knowledge about cases that may become the subject of legal action. Moreover, physicians should learn to distinguish whether skin lesions originate from trauma or not. Key Words: Stretch marks, Physiologic striae, Physical child abuse, Forensic medicine.
