Blockade of glutamate release by botulinum neurotoxin type A in humans: a dermal microdialysis study.
The analgesic action of botulinum neurotoxin type A (BoNTA) has been linked to the blockade of peripheral release of neuropeptides and neurotransmitters in animal models; however, there is no direct evidence of this in humans. ⋯ The present study provided the first direct evidence supporting the inhibitory effect of BoNTA on glutamate release in human skin, which is potentially responsible for some of the analgesic action of BoNTA.

In clinical practice, children are often asked to rate their pain intensity on a simple 0 to 10 numerical rating scale (NRS). Although the NRS is a well-established measure for adults, no study has yet evaluated its validity for children with chronic pain. ⋯ The present study provides preliminary evidence that the NRS is a valid measure for assessing pain intensity in children with chronic pain.

Increasing attention is being devoted to cognitive-behavioural measures to improve interventions for chronic pain. ⋯ The CSQ-R was successfully translated into Italian. The translation proved to have good factorial structure, and its psychometric properties are similar to those of the original and other adapted versions. Its use is recommended for clinical and research purposes in Italy and abroad.

Autonomic arousal frequently is assumed to be a component of the pain response, perhaps because physiological mechanisms connecting pain and autonomic reactivity can be easily conceptualized. The evidence clarifying autonomic responses specific to painful stimulation, however, has been rather sporadic and lacks coherence; thus, a summary and critical review is needed in this area. ⋯ Autonomic arousal can be legitimately measured and modified as one facet of the pain response. Future research should particularly focus on increasing sample size and broadening the diversity of participants. To improve the ability to compare and contrast findings across studies, as well as to increase the applicability of laboratory findings to naturalistic pain, investigators also must enhance experimental design by increasing uniformity or accounting for differences in methodology. Finally, further work remains to utilize more specific assessments of autonomic response and to assess relationships of autonomic reactivity with other cognitive (e.g., attention) and affective (e.g., anxiety) variables.

Pain is very common among nursing home residents. The assessment of pain is a prerequisite for effective multiprofessional pain management. Within the framework of the German health services research project, 'Action Alliance Pain-Free City Muenster', the authors investigated pain assessment adherence according to the German national Expert Standard for Pain Management in Nursing, which is a general standard applicable to all chronic⁄acute pain-affected persons and highly recommended for practice. ⋯ Essentially, pain assessment appeared to be feasible in the majority of the German nursing homes studied. However, the absence or inconsistency of reassessment schedules indicate that pain management guidelines should include a detailed and explicit reassessment schedule for the heterogenic needs of nursing home residents. For residents with severe cognitive impairment, assessment tools are needed that are simple to use and clearly indicate the presence or absence of pain.
