ReviewTeenage pregnancy in the United Kingdom: are we doing enough?
The teenage pregnancy unit's strategy for dealing with the increasing problem of unwanted pregnancy is rather prescriptive and simplistic for such a complex problem. It is not surprising that despite its recent recommendations, the problem has worsened. ⋯ This paper reviews the available evidence on the subject and highlights some good practices from other parts of the western world, which have not been incorporated into the UK strategy. It challenges the various stakeholders to show better commitment by providing a more rigorous and effective strategy.

Case ReportsGynaefix frameless IUD: Cause of bowel resection.
A case of a lost GyneFix intrauterine contraceptive device (IUD) is described, in which laparoscopy failed to identify the device and laparotomy had to be carried out to remove the IUD, which was embedded in the small bowel necessitating bowel resection. Awareness of this complication is necessary, and advanced training is required in order to minimize risks. A description of the GyneFix device, the possible adverse effects and incidence of complications, the importance of post-insertion follow-up, and the need for awareness of the possibility of migration through the bowel are discussed.

Female genital mutilation -- an exported medical hazard.
Female circumcision (FC) has remained a common practice in the countries where it has traditionally been performed. Following increased global mobility, it has also become a common medical issue in the predominantly non-Islamic countries where an increasing number of immigrants from regions where FC is still traditional, have settled. ⋯ FC is performed in immigrant women even after settling in areas where this practise is legally banned. Circumcised immigrant women experience medical and sexual problems which have to be dealt with in their new domicile country. Many African Islamic women, who have migrated to Scandinavia, seem still to be in favour of the continuation of circumcision for varying reasons.

Minimizing missed opportunities: an approach to decrease the unmet need for family planning.
The aim of the study was to determine the participants with unmet need and to show that family planning consultancy given to women and men attending the first-level health facilities would increase contraceptive usage and reduce unmet need for family planning. ⋯ Linking family planning services with other health services can be a cheap and practical way to reduce the unmet need.

Case ReportsEctopic gestation following emergency contraception with levonorgestrel.
Reports of ectopic gestation after failure of emergency contraceptive pills have thus far been rare, although the use of various types of emergency contraceptive pills is becoming more and more popular world-wide. We report two cases of ectopic gestation following failure of levonorgestrel as over-the-counter emergency contraceptive pills. ⋯ We recommend to women the full use of established service networks to enhance education and dissemination of information on emergency contraception. Additionally, health providers should advise women very clearly that, if treatment fails, pregnancy could occur, including ectopic gestation.
