Spontaneous colonic mesenteric hemorrhage: report of an unusual case of abdominal apoplexy.
We describe a case of idiopathic abdominal apoplexy secondary to a spontaneous mesocolic bleed. Preoperative computed tomography of the abdomen suggested a ruptured colonic mass at the splenic flexure. ⋯ Idiopathic abdominal apoplexy is rare, the reported mortality has historically been high, and a short hospital stay and rapid, complete recovery are uncommon, although outcomes may be improving in recent years. We present such a case and examine the clinical presentation, management, and prognosis through literature review.

Several reports have implicated nicorandil as a reversible cause of anal ulceration. We have recently commenced a specialist clinic for patients presenting with severe anal ulceration to assess treatment in this difficult group. Recognition of this association may avoid unnecessary surgery. ⋯ Failure to recognize nicorandil as an etiologic factor in the development of anal ulceration, when other potential underlying well-recognized inflammatory or neoplastic processes have been excluded, may lead to unnecessary surgical intervention in a group of high-risk patients. One of our patients had a potentially avoidable abdominoperineal resection. Pharmaceutical manipulation with alternative antiangina medication may induce healing. Pharmacologic manipulation should be coordinated with a physician to minimize precipitation of unstable angina.

Sphincter-preserving surgery is technically feasible for many rectal cancers, but functional results are not well understood. Therefore, the purpose of this study was to develop an instrument to evaluate bowel function after sphincter-preserving surgery. ⋯ Patients undergoing sphincter-preserving surgery for rectal cancer have impaired bowel function, and those treated with radiation, coloanal anastomoses, or handsewn anastomoses have significantly worse function. This reliable and valid instrument should be used to prospectively evaluate bowel function after sphincter-preserving surgery in patients undergoing rectal cancer therapy.
