Creating non-divergent field edges using asymmetric collimators and a single isocentre can improve matchline dosimetry owing to decreased reliance on operator skills and avoidance of couch movement. However, asymmetic jaws have an associated tolerance that can cause abutment to be misaligned. The matching area dose for monoisocentric three-beam split fields commonly used in head and neck cancer treatments using mismatched and matched collimators is the subject of this work. ⋯ Although the collimators showed a consistent overlap, a perfect dose distribution could be obtained at the matching area. The three-beam split field technique yielded an 8% overdose at the matchline using matched collimators. In conclusion, an awareness of the effects of the abutting technique and digital display tolerance is necessary to achieve good junction uniformity using asymmetric collimators.

The study was designed to investigate the effect of patient position on the detection of ureteral jets in normal pregnancy. The number of jets was recorded for 1 min using colour Doppler ultrasound in 26 healthy asymptomatic gravid women between 16 weeks and 34 weeks gestation. Each patient was examined in the supine position and in both the right and left lateral decubitus positions. ⋯ However, the difference in the detection rate was significant on only the right side (p=0.016 for the right; p=0.063 for the left). 10 (77%) of 13 patients in whom unilateral absence of ureteral jets was noted had detectable jets in the lateral decubitus position. Our findings demonstrated the utility of the lateral decubitus position in the evaluation of ureters suspicious for obstruction. However, caution is still recommended in diagnosing ureteral obstruction if jets are persistently absent after this manoeuvre.

The potential of intensity modulated radiotherapy (IMRT) to improve the therapeutic ratio in prostate cancer by dose escalation of intraprostatic tumour nodules (IPTNs) was investigated using a simultaneous integrated boost technique. The prostate and organs-at-risk were outlined on CT images from six prostate cancer patients. Positions of IPTNs were transferred onto the CT images from prostate maps derived from sequential large block sections of whole prostatectomy specimens. ⋯ The ratio of increased TCP to NTCP may represent an improvement in the therapeutic ratio, but was dependent on the position of the IPTN relative to the anterior rectal wall. Improvements in prostate imaging and prostate immobilization are required before clinical implementation would be possible. Clinical trials are required to confirm the clinical benefits of these improved dose distributions.

Neuroendocrine tumours of the pancreas are rare and are frequently difficult to demonstrate. Several imaging modalities have been used to demonstrate these tumours, but recent reports have suggested that MRI may have an important role in their localization. We review the spectrum of MRI appearances of pancreatic neuroendocrine tumours.
