Case Reports[Therapeutic approach in hemodynamically stable transmediastinal gunshot wounds].
Transmediastinal gunshot wounds may result in damage to the heart, large blood vessels, esophagus or lung. In hemodynamically stable patients, diagnostic examinations have critical importance and the preferred therapies still have unresolved points. In this paper, we present our experience with five patients, three of whom were operated for transmediastinal gunshot wounds after diagnostic tests; all were hemodynamically stable. Before deciding on operation, diagnostic tests should be performed in hemodynamically stable patients with transmediastinal gunshot wounds.

[Principles for the treatment of cardiac injuries: a twenty-two year experience].
Penetrating cardiac injuries have a rather high mortality and require a rapid diagnosis and emergency surgical intervention. The aim of this study was to present and discuss our experience with penetrating heart wounds. ⋯ The basis for successful management of penetrating cardiac trauma is effective resuscitative measures followed by early detection and definition and emergency surgical treatment of the injury.

The association of microalbuminuria and duration of mechanical ventilation in critically ill trauma patients.
Microalbuminuria, which predicts organ failure, probably predicts the necessity of mechanical ventilation as well as its duration. The primary goal of this study was to determine the incidence and presence of a relationship between microalbuminuria and duration of mechanical ventilation in trauma intensive care unit (ICU) patients. ⋯ The ACR can be used in estimating the risk of prolonged mechanical ventilation, even on the first day of admission of critically ill trauma patients.

Accuracy of sonography in detection of renal injuries caused by blunt abdominal trauma: a prospective study.
This prospective study was conducted to evaluate the accuracy of sonography in detection of renal injuries caused by blunt abdominal trauma. ⋯ In spite of the availability and ease of performance of sonography in evaluation of trauma victims, this imaging modality has low sensitivity in detection of renal injuries and overlooks significant damages. CT scan should be considered as the diagnostic modality in victims of kidney trauma who are hemodynamically stable and have clear indications for renal imaging.

Penetrating spinal cord injuries caused by stab wounds are rare. Such injuries may result from a direct lesion of the neural elements of the spinal cord, cord infarcts or, more rarely, intradural or epidural hematoma. In the present study, two cases with spinal cord and L4 root injuries caused by a knife are presented. ⋯ The second patient referred to our emergency outpatient clinic with a torso injury caused by a sharp knife. The knife was embedded 2-3 cm deep at the wound site at the L3 level and the handle was broken. The patient was immediately operated and the broken and embedded metallic part of the knife was extracted.
