Randomized Controlled Trial Multicenter StudyEfficacy of a Novel Prewarming System in the Prevention of Perioperative Hypothermia. A prospective, randomized, multi-centre study.
Perioperative hypothermia is a common complication during general anesthesia. Although rewarming of patients before surgery has been used as a preventive measure and some guidelines recommend it, the implementation of prewarming for every surgical patient is cumbersome. Therefore, we sought to determine the efficacy of two novel prewarming methods that could facilitate prewarming in daily practice. ⋯ In our study active prewarming with a forced-air warmer and an insulating prewarming suit achieves significantly higher core temperatures during anesthesia and at the end of surgery and avoids hypothermia at the end of surgery compared to commercial or conventional insulation techniques.

Randomized Controlled TrialApplication of current guidelines for chest compression depth on different surfaces and using feedback devices: a randomized cross-over study.
Current cardiopulmonary resuscitation (CPR)-guidelines recommend an increased chest compression depth and rate compared to previous guidelines, and the use of automatic feedback devices is encouraged. However, it is unclear whether this compression depth can be maintained at an increased frequency. Moreover, the underlying surface may influence accuracy of feedback devices. We investigated compression depths over time and evaluated the accuracy of a feedback device on different surfaces. ⋯ Strategies are needed to improve chest compression depth, and more than two providers should alternate with chest compressions. The underlying surface does not necessarily adversely affect CPR performance but influences accuracy of feedback devices. Accuracy is improved by a second, posterior, sensor.

Randomized Controlled TrialThe effect of ondansetron on postoperative delirium and cognitive function in aged orthopedic patients.
Postoperative delirium and cognitive dysfunction are common in hospitalized patients. The aim of this study was to investigate whether postoperative ondansetron administration has a favorable effect on postoperative delirium and 30th day cognitive function and pain in patients undergoing surgery with general anaesthesia due to femoral or hip fracture. ⋯ The postoperative ondansetron administration seems to protect and might improve the cognitive function in patients undergoing surgery under general anesthesia. Ondansetron also seems to release analgesic effects.

Randomized Controlled TrialDopamine infusion and fluid administration improve renal function during laparoscopic surgery.
Previous studies have documented the negative influence of pneumoperitoneum on cardiac and renal function during laparoscopy. In this respect, regard there is a lack of data regarding the use of an appropriate management of fluid therapy to decrease these side effects. The aim of this study was to investigate the cardiac and renal responses to the administration of different amounts of crystalloid solution, alone or in association with dopamine during laparoscopy. ⋯ A low rate of saline infusion could lead to an impaired renal function during laparoscopic surgery. This could be prevented by increasing the amount of saline solutions infused per hour or by adding a dopamine infusion.
