Review Case ReportsNecrotizing soft tissue infections of the head and neck: case reports and literature review.
A small but nevertheless important part of a surgeon's experience comprises necrotizing soft tissue infections of the head and neck. The purpose of this report is to heighten awareness of necrotizing soft tissue infections in any patient with an infection of the head and neck. ⋯ Prompt diagnosis and early radical surgical debridement are significant factors in avoiding a fatal outcome in these patients. This article reviews the literature on necrotizing soft tissue infections of the head and neck and presents cases from our recent experience.

ReviewManagement of epistaxis in the oral and maxillofacial surgery setting: An update on current practice.
Epistaxis is a common and, in most cases, benign event. Although most nosebleeds resolve spontaneously, some may be profuse and life-threatening. Severe or recurrent epistaxis can be a challenging management problem. ⋯ The etiology and relevant surgical anatomy are discussed. This is followed by an update on current treatment regimens in different scenarios. A stepwise algorithm for the management of epistaxis is presented.

Thymidylate synthase and dihydropyrimidine dehydrogenase expression in oral squamous cell carcinoma: an immunohistochemical and clinicopathologic study.
Thymidylate synthase (TS) is the target enzyme for 5-fluorouracil (5-FU), and dihydropyrimidine dehydrogenase (DPD) is the first enzyme that metabolizes 5-fluorouracil. Until now, only the enzyme activities of TS and DPD have been investigated; however, there are few reports about the immunohistochemistry of TS and DPD and none regarding oral carcinoma. The purpose of this article was to investigate the expression of TS and DPD in oral squamous cell carcinoma. ⋯ TS may reveal tumor cell proliferation, but DPD-LI may correlate with a response to anticancer drug treatment.

Comparative StudyTissue-dissolving capacity and antibacterial effect of buffered and unbuffered hypochlorite solutions.
The goal of this study was to compare the dissolving potential of Dakin's solution with that of equivalent buffered and unbuffered sodium hypochlorite solutions on fresh and decayed tissues. In addition, the antimicrobial effect of Dakin's solution and equivalent unbuffered hypochlorite was tested. ⋯ In contrast to earlier statements, the results of this study do not demonstrate any benefit from buffering sodium hypochlorite with sodium bicarbonate according to Dakin's method. An irrigation solution with less dissolving potential may be obtained by simply diluting stock solutions of NaOCl with water.

BIS monitoring during midazolam and midazolam-ketamine conscious intravenous sedation for oral surgery.
The purpose of this study was to determine whether the bispectral index scale (BIS) would provide added benefit to established methods of monitoring conscious sedation with midazolam (M group) or midazolam supplemented with ketamine (MK group). ⋯ BIS levels remained close to baseline levels, suggesting that BIS would not provided any additional benefit to currently established methods of monitoring patient consciousness during conscious sedation for oral surgery.
