Incidence and natural history of touch allodynia after open carpal tunnel release.
Open surgical decompression is believed to be a safe treatment with few complications. However, it was our subjective impression that its morbidity had been underestimated. Fifty one consecutive patients with carpal tunnel syndrome were evaluated prospectively for three years after operation. ⋯ These were confirmed by significantly lowered pressure-pain thresholds over both the thenar and hypothenar eminences (p < 0.005). During the first month after operation all patients were relieved of nocturnal pain, and all clinical signs had disappeared at three months in all 51 patients. Our results confirm that open carpal tunnel decompression has a high success rate, but highlights a previously underestimated morbidity of postoperative allodynia.

Effects of lipo-prostaglandin E1 on random pattern flaps in rabbits.
The effects of lipo-prostaglandin (PG) E1, which are lipid microspheres containing PGE1, were examined using 2 x 12 cm random pattern skin flaps on the backs of five rabbits. Five rabbits acted as controls. ⋯ Blood flow was improved five to 15 minutes after injection of lipo-PGE1 and was maintained for at least 60 minutes. In conclusion, lipo-PGE1 increased the blood flow of the random skin flaps, resulting in significant enlargement of the surviving area of skin flaps.

Effect of beraprost sodium (Procylin), a stable prostaglandin I2 analogue, on a dorsal skin flap model in rats.
Beraprost sodium, a chemically stable prostaglandin I2 analogue with antiplatelet vasodilator actions, was tested by comparing the extent of viability of random pattern skin flaps in control rats and rats treated with beraprost sodium. An isolated tail based 2 x 7 cm flap was constructed in 50 Sprague-Dawley rats. ⋯ At the end of the seventh day the flap survival area was documented using digital planimetry. The difference between the results showed that beraprost sodium had significantly beneficial effect on skin flap survival (p < 0.01, Welch's test).

Clinical TrialA new device for securing meshed split-thickness skin grafts.
This report describes the design, operation, and biomechanical performance of the Auto Suture Multifire Graftac-S disposable surgical staplers and absorbable tacks. The performance of this reloadable stapler has been compared to that of the Auto Suture Multifire Premium disposable skin stapler. The Premium stapler forms stainless steel staples to close the wound. ⋯ In two clinical trials of 10 patients each, the Graftac-S delivered absorbable tacks which were biocompatible and successfully secured the graft to the wound. The most obvious advantage of this device is that it obviates the need to remove the staples from the wound later. By the tenth postoperative day, about 90% of the tacks had extruded spontaneously, thereby reducing the amount of postoperative care required; discomfort to the patient during removal of the stainless steel staples is eliminated.

Randomized Controlled Trial Comparative Study Clinical TrialAlkalisation of lignocaine-adrenaline reduces the amount of pain during subcutaneous injection of local anaesthetic.
A double blind, randomised controlled trial was performed to compare the amount of pain experienced after subcutaneous infiltration of lignocaine with adrenaline, and the same anaesthetic buffered to pH 7.1-7.3, in patients undergoing liposuction (n = 8), blepharoplasty (n = 7), mammaplasty (n = 6), and in volunteers (n = 10). A linear analogue pain scale was used to access pain. In all four groups the buffered solution caused significantly less pain (p < 0.001 except for mammaplasty--p = 0.02).
