Comparative Study
Comparison of the Effect of Single Lumbar Transforaminal Epidural Steroid Injections for the Treatment of L4-5 and L5-S1 Paramedian Disc Herniation.
To compare the outcomes of fluoroscopically guided transforaminal epidural steroid injections between L4-L5 paramedian disc herniation and L5-S1 paramedian disc herniation for the treatment of radicular pain. ⋯ This study showed that transforaminal epidural steroid injections for L4-5 paramedian disc herniation were more effective than transforaminal epidural steroid injections for L5-S1 paramedian disc herniation in the 12-week follow-up period.

Orbital Arteriovenous Fistula around the Optic Nerve Treated by Transvenous Embolization.
Intraorbital arteriovenous fistula (AVF) is a very rare disease; therefore an optimal treatment strategy has not yet been established. Here we describe the use of high-resolution cone-beam computed tomography (CBCT) and selective angiography to visualize the detailed anatomy of orbital vessels. The information provided by the intraoperative imaging led to a change in treatment strategy and eventually to successful transvenous coil embolization. ⋯ Superselective angiography and high-resolution CBCT with diluted contrast medium clarified that feeders of the AVF came from the first segment of the ophthalmic artery, and the shunt located surrounding an optic nerve sheath. Transvenous embolization via IOV with coils occluded the AVF. Detailed analysis of intraorbital vessels using high-resolution CBCT and selective angiography led to successful treatment by transvenous coil embolization.

To assess the health-related quality of life (HRQoL) with the Parkinson"s Disease Questionnaire 39 (PDQ-39) after unilateral subthalamic deep brain stimulation (STN DBS) and to identify correlations between the changes in UPDRS (Unified Parkinson"s Disease Rating Scale) scores and separate PDQ-39 QoL dimensions and PDQ summary index (SI) score at short-term follow-up (FU1) and long-term follow-up (FU2). ⋯ Improvements in PDQ-39 dimensions and PDQ-39 SI score are maintained at FU1 and except for the aspects of psychosocial functioning at FU2. Different correlations between the improvements of separate UPDRS scores on PDQ-39 dimensions require future studies in larger study groups.

To assess biomechanical problems related to pedicle screw (PS) systems. ⋯ Excessive bending rigidity and rotational instability are the biomechanical problems related to PS systems. Based on these results, we speculate that one of the most significant causes of adjacent segment disease is excessive bending rigidity and one of the most important causes of instrumentation failure is rotational instability.

To determine the neuroprotective functions of quercetin and compare them with methylprednisolone in an experimental spinal cord injury model in rats. ⋯ The results of this experimental study suggest that quercetin can be thought as an option of treatment in spinal cord injury.
