Intraoperative ultrasound displays dynamic processes intraoperatively. Performing burr-hole biopsies under a real-time visual control is an interesting option for the neurosurgeon. However, the percentage of conclusive diagnoses obtained by this technique and the rate of complications must be evaluated in a larger series. ⋯ When intraoperative ultrasound-navigated biopsies were used they obtained a similar percentage of conclusive diagnoses as stereotactic biopsies. The complication rate is comparable as well. Emerging intracranial complications such as hemorrhages can be observed. However, their incidence cannot be decreased.

Temporary artery occlusion (TAO) during intracranial aneurysm surgery is an integral element in facilitating aneurysm dissection and clipping. Despite its significance, knowledge of effects of TAO on long-term clinical outcome is limited. The purpose of this study was to evaluate the impact of TAO in patients with subarachnoid hemorrhage (SAH) at one institution. ⋯ Limited duration of TAO during aneurysm surgery did not affect long-term clinical outcome and appears to be safe in patients with aneurysmal SAH. Established SAH risk factors including Hunt and Hess grades, Fisher computed tomography grades, and presence of clinical vasospasm clearly correlated with long-term clinical outcomes.

Although the healing response after carotid balloon injury and carotid patch angioplasty injury has been well-documented in animal models, there is limited information about this process after carotid endarterectomy (CEA) in human patients. ⋯ This case report is the first to describe an entire carotid artery specimen in the acute stage after CEA with patch angioplasty. These findings suggest that an α-SMA-rich neointima with prominent proliferative activity covers the inner surface of the treated carotid artery, but patch grafts are left uncovered. The intrinsic arterial wall may have an important role in intimal regeneration after CEA, although the nature of the neointima and the mechanism by which it regulates proliferative activity remain unclarified.
