Randomized Controlled TrialSevere hypertension in pregnancy: hydralazine or labetalol. A randomized clinical trial.
The objective was to compare the safety and efficacy of intravenous labetalol and intravenous hydralazine for acutely lowering blood pressure in pregnancy. ⋯ This randomized clinical trial shows that labetalol and hydralazine fulfill the criteria required for an antihypertensive drug to treat severe hypertension in pregnancy.

Insufficient efficacy of intravenous ketanserin in severe early-onset pre-eclampsia.
To analyze the efficacy of intravenous ketanserin in controlling blood pressure of severe early-onset pre-eclamptic patients. ⋯ Intravenous ketanserin lacks antihypertensive efficacy in a substantial proportion of severe pre-eclamptic patients, despite high dosages. In patients who initially respond well to ketanserin treatment, additional antihypertensive treatment is often necessary to maintain adequate blood pressure control.

Randomized Controlled TrialIs inferior dissection of the rectus sheath necessary during Pfannenstiel incision for lower segment Caesarean section? A randomised controlled trial.
The objective was to determine the benefit of non-dissection of the rectus sheath inferiorly in a Pfannenstiel incision during an elective Caesarean section with regard to operative blood loss and post-operative pain. ⋯ Non-dissection of the rectus sheath inferiorly in Pfannenstiel incisions during Caesarean section procedures is associated with a significant reduction in the post-operative pain as well as a smaller drop in post-operative haemoglobin.

Obstetric intensive care admissions: a 12-year review in a tertiary care centre.
To review all pregnant women who required admission to an Intensive Care Unit (ICU) during pregnancy, childbirth or puerperium. ⋯ We need better information about high-risk obstetric patients in order to prevent severe maternal morbidity and to improve maternal care. The high number of non-caucasian women requiring ICU admission indicates the need for a study into the role of ethnicity. We have initiated a nationwide confidential enquiry into the causes of severe maternal morbidity.

Arterial stiffness in preeclamptic and chronic hypertensive pregnancies.
To assess arterial stiffness in pregnancies complicated by hypertensive disorders: preeclampsia and chronic hypertension. ⋯ Besides the vasoconstriction of smaller peripheral arteries, our finding of increased arterial stiffness in preeclamptic pregnancies suggests that also larger arteries with altered viscoelastic properties are involved in the aberrant hemodynamics of preeclampsia. Compared to preeclamptic subjects, women with chronic hypertension shared a common feature of high systemic vascular resistance, but changes in arterial stiffness were less than in preeclampsia.
