The preparedness of pediatricians for emergencies in the office. What is broken, should we care, and how can we fix it?
To determine the frequency of emergencies in pediatric practices, assess office emergency preparedness, and explore practitioner's reasons for levels of preparedness. ⋯ Emergencies are common in pediatric practices, but problems in the emergency preparedness of practices persist despite previous studies with similar findings, and publication of guidelines. Perceptions of practitioners suggest that not enough effort has been devoted to increasing practitioner awareness of these problems. Greater awareness might result by emphasizing practical instruction in residency and continuing medical education courses, and by publication of guideline summaries.

Covariations of unhealthy weight loss behaviors and other high-risk behaviors among adolescents.
To determine if unhealthy weight loss methods are associated with other health-compromising behaviors among adolescents and to examine covariation patterns across gender and age groups. ⋯ Adolescents who engage in unhealthy weight loss methods are more likely to engage in a range of other health-compromising behaviors. Different perceptions of unhealthy weight loss behaviors (eg, normative vs problematic) may in part explain the differences in the strengths of associations between different grade and gender groups. Our results suggest that screening and counseling of adolescents who engage in unhealthy weight loss methods should be comprehensive and intervention programs aimed at the secondary prevention of disordered eating need to address other problematic behaviors.

Randomized Controlled Trial Clinical TrialSucrose as an analgesic agent for infants during immunization injections.
To assess the effectiveness of sucrose as an analgesic agent during routine immunization injections for infants (age range, 2 weeks to 18 months). ⋯ We found that when infants drank sucrose or sterile water, significantly fewer pain vocalizations were produced, but only for 2-week-old infants. For older infants, differences were found only when the number of injections was included in the analysis. We expand on previous findings by demonstrating that both the age of the child and the number of painful exposures can attenuate calming effects. In addition, the results suggest that in the absence of nonnutritive sucking, the actual analgesic effects of sucrose may be nonspecific. Further study is needed of the possible analgesic effects of sucrose.

Multicenter StudyGun storage patterns in US homes with children. A pediatric practice-based survey. Pediatric Practice Research Group.
To describe gun storage patterns in gun-owning families with children. ⋯ Because most gun-owning families store guns loaded, unlocked, or both, anticipatory guidance should address gun storage in all such families. Interventions designed to alter the way work guns are dealt with after work, and to provide safe and effective means of self-protection might affect these storage patterns.

To improve characterization and recognition of inflicted pediatric submersions. ⋯ Enough childhood submersions are inflicted to warrant careful case evaluation. Bathtub submersion victims and children with physical and historical findings common to other forms of abuse are most likely to be the victims of inflicted submersion.
