To describe the clinical characteristics of patients diagnosed with spontaneous abdominal rectus muscle hematoma (RSH), predisposing and precipitating factors, and therapeutic management. ⋯ RSH is more frequent in elderly patients, treated with acenocumarol and in the supratherapeutic range, and frequently entails permanent suspension of anticoagulant therapy. Renal insufficiency is related to the overdosage of the anticoagulant treatment and to the production of the hematoma. The size of RSH and transfusion requirements are factors that seem to be related to a greater need for invasive treatment through arterial embolization or surgery. Key words. Multicenter Study Observational Study[Good care for patients who die in intensive care units in Spain. A study based on international care quality indicators].
Good care for patients who die in intensive care should be pursued in the same way that excellence is sought in other clinical aspects. ⋯ The quality of end-of-life care in the participating ICU needs to be improved. The study identifies shortcomings and indicates existing resources in clinical practice from which a gradual improvement plan, adapted to the situation in each hospital, can be designed. The analysis, inexpensive in its implementation, offers an opportunity for improvement, a goal recommended by most professional societies of intensive care medicine. Key words. End-of-life care. Observational Study[Influence of sociodemographic, occupational and life style factors on the levels of burnout in palliative care health professionals].
Palliative care health professionals have reported high levels of burnout. An understanding of factors associated with this syndrome in this population could foster new prevention and intervention strategies. The objectives were to evaluate the levels of burnout in each of its dimensions in a sample of palliative care health professionals and to analyze the relationship between levels of burnout and sociodemographic, occupational and lifestyle characteristics in this sample. ⋯ The younger professionals, with more job seniority in the units of palliative care, and less hours of sleep and physical activity showed higher levels of burnout. Therefore, these factors should be considered for enhancing prevention and intervention strategies for these health professionals.

Case ReportsCongenital obstruction of the foramen of Monro causing unilateral hydrocephalus. A propos of a case
Isolated hydrocephalus of the lateral ventricle may result from unilateral occlusion of the foramen of Monro. The most common causes are tumours of the lateral ventricles or in the area of the third ventricle. ⋯ The patient underwent endoscopic fenestration of the foramen of Monro and septostomy with good postoperative clinical evolution and reduction of ventricular size. Intraoperative findings demonstrated membranous obstruction of the right foramen of Monro. A successful treatment can be accomplished through a neuroendoscopic approach avoiding the use of ventricular shunts.

The organizational commitment of emergency physicians in Spanish public hospitals
Background. There are not too many studies that deal with the organizational commitment of emergency physicians. This commitment has been shown to impact organizational performance. ⋯ The capabilities of the physician have an influence on their affective commitment; specific training in emergency procedures and seniority has an influence on their continuance commitment; and the opinion they hold about the organization of their service influences affective commitment. Conclusions. Emergency physicians show an average involvement in the hospital in which they work (average 3.8 on a range of 1 to 5), feel an average affection for it (3.4), and have a high intention to keep working there (4.0). The resources the hospital has due to its level do not have an influence on this commitment, while the training and perceptions of the service do have an influence.
