Isolation of the airway sometimes determines the survival or death of the patient. To anticipate the presence of a difficult airway (DA) there are a number of indicators that are validated for hospitals: Mallampati, sternum and thyromental distance, interdental distance and Cormack grade. The aim of this study is to evaluate the use of these indicators in the ambulatory setting and to know the incidence of DA. ⋯ Interdental distance was less than 3 cm if presence is associated with DA (p = 0.005). The sensitivity and specificity of all measures are low. According to our series the sternum and thyromental distance are not useful in the ambulatory setting, but interdental distance is useful for predicting a DA.

[Characteristics and epidemiological changes for patients with community-acquired pneumonia in hospital emergency departments].
The aim of this paper is to determine both the prevalence of community-acquired pneumonia (CAP) in the A & E Department and the most relevant epidemiological changes in the last decade, as well as the profile and management of these patients. ⋯ The impact and prevalence of CAP in A & E Departments has increased in the last decade. It is the most common cause of sepsis, severe sepsis and septic shock, admission to intensive care units and death due to infectious disease.

Case Reports[Hemomediastinum caused by rupture of a bronchial artery aneurysm].
Hemomediastinum caused by rupture of a bronchial artery aneurysm is a very infrequent pathology. Clinical presentation can be variable; it can resemble an aortic pathology and be diagnosed in emergency situations. ⋯ TC was carried out with a diagnosis of suspected mediastinal neoplasia (Hounsfield Units/HU 38 - soft tissue), biopsy sampling was carried out by gastroscopy and broncoscopy with haemorrhagic pap smear, presenting light anemization on admission and remaining haemodynamically stable and asymptomatic. Facing a high clinical suspicion of arterial pathology, CT was repeated in which hemomediastinum was described in resolution, with subsequent confirmation of the existence of ruptured bronchial artery aneurysm through angiography and proceeding to therapeutic embolization.

Review[Review of initiatives carried out in Spain to implement teaching of basic cardiopulmonary reanimation in schools].
Cardiorespiratory arrest (CRA) is a situation of maximum medical emergency which can be potentially reversed if basic cardiopulmonary resuscitation manoeuvres (CPR) are provided by the first bystander. Nonetheless, this is done in less than 25 % of the cardiorespiratory arrests (CRA) witnessed by bystanders. ⋯ One strategy consists in the teaching of basic CPR techniques in schools following the recommendations made by specialized institutions. This study analyzes the initiatives carried out in Spain, including the authors' own experience acquired during the development of PROCES, a program aimed a students from 15 to 16 years of age and carried out during the last 10 years in Barcelona.

Aspirin and other non-steroidal anti-inflammatory drugs (NSAIDs) have been associated with a reduction in the risk of developing adenomas and colorectal cancer. The available scientific evidence justifies the need for more studies that evaluate the protective effect of aspirin and other NSAIDs as a preventive measure against cancer of the colon; define the minimum efficient dosage; the age at which to begin treatment; the most convenient duration of the latter; and the sub-populations for which the benefits of chemoprophylaxis outweigh possible adverse effects.
